I. GEORGIA MUNICIPAL ASSOCIATION (GMA) CITY OF ETHICS PROGRAM:



Authorization is requested to apply for recertification and, if approved,

accept recognition from the Georgia Municipal Association as a "Certified City

of Ethics". An agenda report and resolution are attached. Click link to

II. COLUMBUS BOARD OF HEALTH - SMART CHOICE PROGRAM:



Authorization is requested to submit an application and if approved accept

$6,000 from the Columbus Board of Health Smart Choice Program for a period of

three months to purchase equipment and informational materials designed to

address the issues of childhood obesity. An agenda report and resolution are

III. JUDICIAL COUNCIL OF GEORGIA - ADULT DRUG COURT GRANT:



Authorization is requested to submit an application and if approved

accept a grant from the Judicial Council of Georgia to fund the Muscogee County

Adult Drug Court for a one year period beginning July 1, 2009. An agenda

Scope of RFB

This work shall consist of the installation of a new storm drain in the

vicinity of Cora Drive, all in accordance with the designated plans and

specifications. The Contractor shall be required to maintain public access to

all residences located within the project area.



March 13, 2009

1. Food & Beverage Management Services for Columbus Civic Center (Annual

Contract) ? RFP No. 09-0014

Scope of RFP

Provide all equipment and personnel necessary for the food and beverage

management services at the Columbus Civic Center. The term of this contract

shall be for two years, beginning July 1, 2009, with the option to renew for

three additional one year periods. Contract renewal shall be contingent upon

the mutual agreement of the City and the Contractor.

1. Ball Field Fencing ? Britt David Park (Re-Bid) ? RFB No. 09-0032

Scope of RFB

The scope of work includes, but is not limited to, the removal of existing ball

field fencing at one field and the installation of a new fence system for same

in accordance with the specifications. The field is more identified within the

park as Little League Field Number Two. Measurements are the responsibility of

the bidder.

3. Storm Drain Rehabilitation Priority 1 Pipes ? Contract 3

Scope of RFB

The project consists of the following major elements: performing

cured-in-place storm sewer repairs as follows: 355 feet of 15 to 18-inch pipe,

1,635 feet of 20 to 24-inch pipe, 1,165 feet of 30 to 36-inch pipe, 1,125 feet

of 36-inch pipe, 395 feet of 42-inch pipe, and 375 feet of 48-inch pipe.

Performing CCTV inspection of 430 feet of 12 to 15-inch pipe, 460 feet of 20 to

24-inch pipe, 870 feet of 30 to 36-inch pipe, 2,400 feet of 42 to 48-inch pipe,

and 660 feet of 18 X 36-inch pipe. Miscellaneous point repairs, manhole height

adjustments and installation of manholes. The City will only consider bids

from pre-qualified contractors.

1. Material Recovery Facility (MRF) Operations (Annual Contract) ? RFP No.

09-0016

Scope of RFP

Columbus Consolidated Government is seeking proposals from qualified vendors to

provide Material Recovery Facility Operations to the Columbus Consolidated

Government Recycling Division. This proposal should allow the City of Columbus

Recycling Division to deliver all recycling materials collected to the vendor

for processing and sale. The term of the contract will be for one (1) year

with the option to renew for four (4) additional twelve (12) month periods.



March 25, 2009

1. 30? Foot Low Floor Trolley Bus (Annual Contract) ? RFB No. 09-0034

Scope of Bid

The Consolidated Government of Columbus, Georgia (the City) is seeking vendors

to provide 30? Foot Low Floor Trolley Buses to METRA on an ?as needed? basis.

The City anticipates purchasing approximately 0-5 buses per year over a five

(5) year period.



April 1, 2009

1. Warm Springs Bike Trail ? Phase One ? RFB No. 09-0037

Scope of RFB

This work shall consist of the construction of a multi-purpose concrete/asphalt

trail, designated as Bike Trail, all in accordance with the plans and

specifications. The Contractor shall be required to maintain public access to

2. Hybrid Low Floor Electric Buses (Annual Contract) ? RFB No. 09-0038

Scope of RFB

The Consolidated Government of Columbus, Georgia, is seeking vendors to provide

Hybrid Low Floor Electric Buses to Department of Transportation/METRA on an ?as

needed basis?. This contract will be a five (5) years with the intent to

purchase 0-5 buses per year. Contract renewal will be contingent upon the

mutual agreement of the City and the Contractor.
