I. ADULT FELONY DRUG COURT GRANT:



Authorization is requested to submit an application and if approved

accept funds from the Judicial Council of Georgia Standing Committee on

Accountability Courts for the operation of the Muscogee County Adult Felony

Drug Court, for treatment of participants with co-occurring disorders in the

amount of $27,745.00. An agenda report and resolution are attached.

II. MEMORANDUM OF UNDERSTANDING CHATTAHOOCHEE VALLEY AREA CHAPTER OF THE

SOUTHERN OFF-ROAD BICYCLE ASSOCIATION (CVA SORBA) - FLAT ROCK PARK PARTNERSHIP:



Authorization is requested to enter into a Memorandum of Agreement with

CVA SORBA to maintain, promote and usage of the existing mountain biking trails

at Flat Rock Park in partnership with the Columbus Consolidated Government. An

III. CONSTRUCTION ADMINISTRATION AND CONSTRUCTION QUALITY ASSURANCE SERVICES

(CQA):



Authorization is requested to enter into an agreement with Jacobs

Engineering Group in the amount of $275,200 to perform CQA activities in

association with the construction of Phase 4 of Pine Grove Municipal Solid

Waste (MSW) Landfill. An agenda report and resolution are attached.

IV. U. S. CONFERENCE OF MAYOR'S GRANT:



Authorization is requested to accept $4,000 in grant funds from the U. S.

Conference of Mayor's for "Dollarwise - Summer Youth Campaign 2012" Grant and

execute a Memorandum of Understanding with Turnaround Columbus for Workforce

Investment Act (WIA) Department to administer the grant. An agenda report and

Scope of RFP

Construct a new recycling facility on Pine Grove Road. The work shall consist

of, but shall not be limited to, a 44,674 square foot process building, a 9,668

square foot sustainability center, truck scales, truck docks, lift stations,

grading, storm drainage, erosion and sedimentation control, landscaping and

irrigation, detention pond, asphalt and concrete paving, parking areas, and

fencing.

12-0020

Scope of RFP

Seeking qualified financial institutions to provide merchant services for

credit card payment processing. Contract term shall be for two years, with the

option to renew for three (3) additional twelve-month periods.



2. Right of Way Acquisition Services (Annual Contract) ? RFP No. 12-0021

Scope of RFP

Seeking multiple consultants to provide Right of Way Acquisition Services.

Contract term shall be for two years, with the option to renew for three (3)

1. Corporate Ridge Parkway at Woodruff Farm Road Intersection Improvements ?

RFB No. 12-0044

Scope of Bid

The contractor shall follow the Georgia Department of Transportation (GDOT)

Standards and Specifications, in conjunction with Columbus Water Works for

installation of all water and sewer items. The contractor shall follow the

GDOT Standards and Specifications for miscellaneous erosion control items;

maintenance of control devices, water quality monitoring; silt fence; highway

signs; thermoplastic striping; grassing; patching concrete bridge; temporary

barrier; and traffic control.

Scope of RFP

Provide ticketing services for Columbus Consolidated Government Civic Center on

an ?as needed basis?. Contract term shall be for five (5) years, with the

option to renew for an additional five-year period.

1. Audit Services of Contract for Hospital Care to Indigent Persons ?

RFP No. 12-0014

Scope of Proposal

Columbus Consolidated Government is soliciting proposals for the professional

services of a Certified Public Accountant to perform a compliance audit of the

hospital care to indigent persons contract between the Columbus Consolidated

Government and the Medical Center Hospital Authority.



2. Course Instructors for the Department of Human Resources, Learning Center

(Annual) ? RFP No. 12-0022

Scope of Proposal

Provide Human Resources, Learning Center with instructors in various training

modules to employees interested in growing and developing their skills within

the Columbus Consolidated Government. The initial term of the contract shall

be on a per-course basis, up to one year, with the option to renew for one

additional year. Contract renewal will be contingent upon the mutual agreement

of the City and the Contractor.



3. Lead-Based Paint/Asbestos & Clearance Testing Services (Annual Contract) ?

RFP No. 12-0023

Scope of Proposal

Provide services for lead-based paint/asbestos and clearance testing services

for the housing rehabilitation, rental assistance program, and supportive

services/operations programs. The initial term of the contract shall be for

two years, with the option to renew for three additional twelve-month periods.

Contract renewal will be contingent upon the mutual agreement of the City and

the Contractor.
