I. BENEFITS MANAGER CONTRACT:
Authorization is requested for the approval of a resolution authorizing
a 1-Year Benefits Manager Contract Extension with Northwestern Benefits

II. COLUMBUS YOUTH SOCCER ASSOCIATION (CYSA) MEMORANDUM OF UNDERSTANDING (MOU):
Authorization is requested to execute a Memorandum of Understanding
(MOU) with Columbus Youth Soccer Association to operate all aspects of soccer
programming at the Woodruff Farm Soccer Complex until December 31, 2016 with
the option to renew for an additional 5 years. An agenda report and resolution

III. LEARNING TOGETHER PARENTS AS TEACHERS PROGRAM:
Authorization is requested to submit an application, and if approved,
accept a grant from the Governor's Office for Children and Families (2012
System of Care: Community Strategy Grant Program) for $55,390, includes
$27,695 in grant funds and a required equal match of $27, 695 from the City.

IV. STREET ACCEPTANCE - THAT PORTION OF CUPOLA PLACE LOCATED IN ARBOR POINTE,
PHASE II:
Authorization is requested for the acceptance of a deed to that portion
of Cupola Place located in Arbor Pointe, Phase II. The Department of
Engineering has inspected this street and recommends the acceptance. An agenda

V. STREET ACCEPTANCE - CIMARRON COURT AND THAT PORTION OF BRIDGEMILL DRIVE
LOCATED IN SECTION V, BRIDGEMILL:
Approval is requested for the acceptance of a deed to Cimarron Court
and that portion of BridgeMill Drive located in Section V, BridgeMill. The
Department of Engineering has inspected these streets and recommends the

A Non-Mandatory Pre-Bid Conference/Site Visit is scheduled for Thursday,
February 16, 2012 at 10:30 AM (Eastern). Vendors shall convene in the Scale
House at Pine Grove Landfill, which is located at 7160 Sacerdote Lane.

1. On-Site Clinic for Employees, Retirees, & Dependants (Annual Contract) ? RFP
No. 12-0010
Scope of RFP
Contract with a clinic provider to establish an on-site or nearby clinic for
employees, retirees and dependants. The contract term shall be for two years,
with the option to renew for three additional twelve-month periods. Contract
renewal will be contingent upon the mutual agreement of the City and the
Contractor.

1. Sand Spreaders ? RFB No. 12-0026
Scope of Bid
Provide one (1) 10' Sand Spreader and one (1) 7' Sand Spreader, to be used by
the Right of Way Maintenance Division, of the Public Services Department, to
spread sand during inclement weather. Sand Spreaders must work with existing
trucks: 2009 Peterbuilt 14-Yard Dump Truck and 1997 Ford F&B Series 7-Yard Dump
Truck (dump size 7'x11" outside to outside with an 85-inch bed). Installation
and/or technical assistance on-site may be required.

1. Developer/Operator for an RV Park and/or Marina at Oxbow Meadows
Environmental Park ? RFP No. 12-0008
Columbus Consolidated Government is looking for qualified developers/operators
to submit a proposal for the development of certain City-owned property located
in Oxbow Meadows Environmental Park. The success of the City?s 16-mile
Riverwalk project has prompted interest by the City to target portions of
vacant Oxbow Meadows land for additional community improvements.
2, Performance Management Consulting Services ? RFP No. 12-0016
Columbus Consolidated Government is seeking a consultant (s) to provide
data-based performance review and management services. The Consultant shall
work with departments, boards, commissions and/or agencies to improve their
operations, efficiency and effectiveness, and shall facilitate the process of
cross-departmental cooperation for the purposes of improving the efficiency,
effectiveness and quality of public services in Columbus, Georgia.
The goal for the Consultant Services is to develop database templates and
incorporate strategies for continuous improvement for all departments of the
Columbus Consolidated Government. These strategies include developing database
templates for information and analysis reporting, benchmarking, leadership
assessment, strategic planning, human resource utilization, budgetary and
efficiency assessments, business results and any other purpose that promotes
the efficient and effective use of taxpayer dollars. Incorporation of these
assessment strategies should be best practices for providing services to our
community.
3. Recycling Operations and Maintenance Services (Annual Contract) ?
RFP No. 12-0017
Scope of RFP
Provide recycling processing equipment, provide operations and maintenance of
the equipment and the facility, market the processing capacity of the
facility, market processed recyclable materials, and assist CCG with public
education services. Contract term shall be for ten (10) years, with the option
to renew for five (5) additional twelve-month periods. Contract renewal will be
contingent upon the mutual agreement of the City and the Contractor.

1. Pine Grove Municipal Solid Waste Landfill Phase 4, Cells 4A, 4B, 4C, 4D, 4E,
4F, 4G and 4H ? RFB No. 12-0035
The Project will consist of clearing and grubbing; earthwork (cut/fill) to
subbase elevations; placement of 24-inch thick liner base with permeability not
to exceed 1 x 10-7 cm/sec., or GCL with a 24-inch thick base liner with a
permeability not to exceed 1 x 10-5 cm/sec. and placement of either a 24-inch
thick leachate collection layer with a permeability no less than 1 x 10-2
cm/sec. or placement of geonet and 24 inches of a soil layer with no specified
permeability; a 60 mil HDPE geomembrane liner over 16.1 acres; the construction
of 1,300 feet of 24-foot gravel access road; installation of erosion control
devices; installation of a stormwater protection layer; installation of
underdrain and leachate collection, and force main piping. The City will only
consider bids from pre-qualified contractors.

Scope of Proposal
Columbus Consolidated Government is soliciting proposals for the professional
services of a Certified Public Accountant to perform a compliance audit of the
hospital care to indigent persons contract between the Columbus Consolidated
Government and the Medical Center Hospital Authority.
