I. UNUSED SICK LEAVE PAYMENT:



Authorization is requested for payment to employees for unused sick

leave in accordance with 16B-15-6(2) of the Columbus Code of Ordinances. An

II. AGREEMENT TO QUIT CLAIM DEED OF CITY PROPERTY TO THE GEORGIA DEPARTMENT OF

TRANSPORTATION:



Authorization is requested to execute a Document titled "Department of

Transportation Option for Right of Way" (attached) for a "Quit Claim Deed" with

the Georgia Department of Transportation (GDOT) for $3,000 for property owned

by the city, located at 2000 American Way, and to accept Road Widening of

Veterans Parkway from Old Moon Road to Turnberry Lane, Muscogee County. An

III. UNITED STATES ARMY CHILD YOUTH SCHOOL SERVICE STRONG (CYS)- AFTER SCHOOL

PROGRAM IN YOUR NEIGHBORHOOD (ASPYN):



Authorization is requested to submit an application, and if approved,

accept $187,500 and any additional funds that may be awarded in fiscal years

2013 and 2014 from the United States Army CYS Strong (ASPYN) Program. Funds

will be used to offset direct care equipment and materials, training,

administrative expenses and direct program staff cost at accreditation sites

and any other site with military children enrolled in the Community Schools

Division program (CSD) of the Columbus Parks and Recreation Department. An

IV. STREET ACCEPTANCE - SMOKE DRIVE - RUNNING FROM AIRPORT THRUWAY TO SOWEGA

ROAD:



Authorization is requested for the acceptance a deeds and easement to

Smoke Drive, running from Airport Thruway to Sowega Road. The Department of

Engineering has inspected this street and recommends the acceptance. An agenda

1. Full-Size, 4-Door Sedans (Non-Pursuit) ? RFB No. 13-0024

Scope of RFP

The Columbus Consolidated Government wishes to purchase twenty-one (21)

full-size, 4-door sedans (non-pursuit). Fifteen (15) vehicles will be used by

the Columbus Police Department and six (6) vehicles will be used by the

Sheriff?s Office. These are replacement vehicles.



2. Security Cameras & Accessories for Buses (Annual Contract) ? RFB No.

13-0020

Scope of Bid

Provide security cameras, which will be installed on buses of Metra Transit

System. Approximately 0-28 cameras will be purchased each year, on an ?as

needed basis?. The City will only consider bids for GE Mobile View 3000

Digital Video Recorder, which is the standardized security camera for Metra

Transit System. The contract term shall be for one year with the option to

renew for two (2) additional twelve-month periods.

1. Naming Rights Partnership/The Columbus Civic Center (Annual Contract) ? RFP

No. 13-0016

Scope of RFP

Columbus Consolidated Government is seeking a corporation to be the naming

rights partner for the Columbus Civic Center. Vendors have the option to

schedule a tour of the Columbus Civic Center beginning November 5 ? 23, 2012,

Monday through Friday, between the hours of 9:00 AM ? 3:00 PM. To make an

2. Pursuit Car Build Out (Annual Contract) ? RFP No. 13-0017

Scope of RFP

The Columbus Consolidated Government (The City) requires the services of a

qualified provider to acquire and install the specified equipment into the

pursuit vehicles purchased by the City each year; and prepare them for delivery

to the City of Columbus, as a fully functional pursuit vehicle, for use by one

of the Law Enforcement Agencies of the City of Columbus.

3. Integrated Waste Routing Program ? RFP No. 13-0018

Scope of Bid

The Columbus Consolidated Government (the City) is seeking proposals for

software that will provide us with the ability to evaluate and achieve the best

balance of performance and efficiency for routing vehicles in the performance

of their daily tasks. The City currently provides diversified Waste Collection

services for its citizens that necessitate vehicles navigating city streets for

pickups. Such services include trash pickup, yard waste, bulk waste and

recycling collection. The City services an estimated 56,500 households and

traverses an estimated 3,000 miles per week, all within an area of 220 square

miles. The vendor will provide a comprehensive routing software system using

ESRI based geographic information that shall include, at a minimum, a balanced

routing system that: a) enables the development of an equitable workload for

route drivers; b) improves customer service and production efficiencies; and c)

provides maps and travel direction reports of all routes.

1. Aquatic Facility Management of the Columbus Natatorium (Annual Contract) ?

RFP No. 13-0019

Scope of RFP

The Columbus Consolidated Government is seeking proposals, from qualified

firms, for the management of the Columbus Natatorium, to include: staffing,

water maintenance, supervision and programming; as well as, Pro-Shop and

Concession management. Operation is scheduled to begin June 2013.
