I. TAD # 5 - MIDTOWN WEST:
Approval is requested to execute an Intergovernmental Agreement between
Columbus, Georgia, a consolidated government and the Muscogee County School
District with respect to the TAD in substantially the same form as attached as
Exhibit A.
II. TAD # 6 - MIDTOWN EAST:
Approval is requested to execute an Intergovernmental Agreement between
Columbus, Georgia, a consolidated government and the Muscogee County School
District with respect to the TAD in substantially the same form as attached as
Exhibit A.
III. ACCEPTANCE OF A DEED FOR ADDITIONAL RIGHT-OF-WAY ADJACENT TO OLD DOUBLE
CHURCHES ROAD:
Approval is requested for the acceptance of a portion of property along
Double Churches Road, .038 acres for additional right-of-way for future road
improvements.
IV. ACCESS EASEMENT - FRONT AVENUE:
Approval is requested to execute an easement with W.C. Bradley Company
for access to the property formerly known as "Front Avenue" between 13th Street
and 14th Street for ingress, egress and regress for the general use of the
public with respect to vehicular and pedestrian traffic.
V. RIGHT OF WAY ABANDONMENT - PORTION OF REESE ROAD:
Approval is requested to delete from the "Official Map of Consolidated
Government of Columbus, Georgia" an improved segment of formerly Reese Road
(Old Reese Road) between the south right of way line of Milgen Road along the
eastern right of way line of Reese Road for a distance of approximately 220 ft,
declaring it surplus and authorizing its disposal in accordance with the City
Codes.

Scope of RFP
Columbus Consolidated Government (the City) is requesting proposals from
qualified firms to provide professional architectural, engineering and planning
services on an as-needed basis. The contract term shall be for two (2) years
with the option to renew for three (3) additional twelve-month periods.
2. Supplemental EMS Coverage (Annual Contract) ? RFP No. 17-0010
Scope of RFP
Columbus Consolidated Government (the City) is requesting proposals from state
of Georgia certified emergency medical services vendors to provide supplemental
advanced life support transport services, on an as-needed basis, in support of
Columbus Fire and Emergency Medical Services.

Scope of Bid
Provide trees to be planted on City right-of-ways for Columbus Consolidated
Government?s Department of Public Works. The trees will be purchased on
as-needed basis. The term of the contract shall be for two (2) years, with the
option to renew for three (3) additional twelve-month periods.
2. Tire Leasing Services for City Buses (Annual Contract) ? RFB No. 17-0027
Scope of Bid
Provide tire leasing services for City buses to the Columbus Consolidated
Government Department of Transportation/METRA Transit System, on an ?as-needed?
basis. The term of the contract shall be for two (2) years, with the option to

1. Landscape Improvements ? 2nd Avenue & 18th Street
Scope of Bid
The scope of work includes clearing & grubbing; erosion and sediment control;
planting of Emerald Colonnade Hollies, Muscogee Crape Myrtles, Shoal Creek
Vitexes, Carissa Hollies, Dwarf Buford Hollies, Pink Muhlies, Pizazz
Loropetalums, Coral Drift Roses, Dwarf Yaupons and Meyer Zoysia Sod; install
landscape mulch; and install an irrigation system.

1. Melrose Drive, Boxwood Boulevard & Decatur Street Bridge Replacements and
Waterford Road Bridge Improvements
Scope of Bid
Scope of the project includes removal and replacement of 3 bridges over Lindsey
Creek in Columbus, GA. The bridges are located on Melrose Drive, Boxwood
Boulevard, and Decatur Street. The bridges will have to be constructed
consecutively as detours for each bridge will have to use the others.
Work includes demolition of existing structure, complete installation of new
bridge, utility work as shown on the Contract Documents, and landscaping as
shown on the Contract Documents. Also included in this Bid is the replacement
of the bridge deck on Waterford Road as shown in the Contract Documents.
2. Welding/Metal Fabrication Services (Re-Bid) ? RFB No. 17-0030
Scope of Bid
Provide welding and metal fabrication services for various facilities and
equipment owned by the City. The Term of the contract shall be for two (2)
years, with the option to renew for three (3) additional twelve-month periods.
