I. MEMORANDUM OF UNDERSTANDING (MOU) - GREATER COLUMBUS CHAMBER OF COMMERCE:



Authorization is requested to enter into a Memorandum of Understanding (MOU)

with the Greater Columbus Chamber of Commerce, for the acceptance of

Transportation Enhancement funds from the Georgia Department of Transportation

for restoration and preservation of the historic Columbus Depot for public use

and for the Chamber's offices. An agenda report and resolution are attached.





II. FY2007 GADOT TRANSPORTATION PLANNING CONTRACT:



Authorization is requested to apply for and receive funds from the Georgia

Department of Transportation to perform transportation planning activities

throughout the Columbus-Phenix City urban area. An agenda report and resolution

are attached.





III. ANNUAL ACTION PLAN:



Authorization is requested for the filing of the Annual Action Plan and

applications with the Department of Housing and Urban Development requesting

entitlement funding for the Community Development Block Grant (CDBG), HOME

Investment Partnership (HOME), and Emergency Shelter Grant (ESG) programs. An

agenda report and resolution are attached.





IV. PURCHASES:



Approval is requested of items "A" thru "D":

B. Emergency Rehabilitation of Cherokee Avenue Storm Drain ? Phase One

C. Lowboy Tri-Axle Trailer (Re-Bid)

D. Crime Insurance Coverage ? July 1, 2006 through June 30, 2007





ADD-ON:



V. STREET ACCEPTANCE ? IVY PARK DRIVE AND ENGLISH IVY DRIVE:



Authorization is requested to accept a deed to Ivy Park Drive and English Ivy

Drive located in Phase I, Ivy Park. The Department of Engineering has inspected

these streets and recommends the acceptance. An agenda report, map and

resolution are attached.





VI. RESCIND RESOLUTION NO. 213-06 - QUIT CLAIM DEED, SOUTHERN STREET:



Authorization is requested to rescind Resolution # 213-06 to declare the

portion of street between Comer Avenue and 11th Ave as surplus and execute a

Quit Claim Deed. This resolution was adopted in error. A resolution to

rescind is attached.

1. Long Distance Telephone Services (Annual Contract) ? RFP No. 06-0015

Scope of RFP

Provide Columbus Consolidated Government with long distance telephone services.

The initial term of the contract shall be for two years beginning July 1, 2006,

with the option to renew for three (3) additional twelve-month periods.

Contract renewal will be contingent upon the mutual agreement of the City and

the Contractor.

1. Back Support Safety Belts & Safety Vests (Annual Contract) ? RFB No. 06-0085

Scope of Bid

Provide approximately 200 back support safety belts and approximately 3,000

safety vests to Columbus Consolidated Government (the City) on an "as needed"

basis. The contract term will be for two years, with the option to renew for

three additional twelve-month periods.



2. Plumbing & Irrigation Supplies (Annual Contract) ? RFB No. 06-0087

Scope of Bid

Provide plumbing & irrigation supplies on an ?as needed basis? to be picked up

and delivered to Columbus Consolidated Government. The contract term will be

for two years, with the option to renew for three additional twelve-month

periods.
