I. FY2017 HOLIDAY SCHEDULE:
Approval is requested to approve the FY2017 Holiday Schedule
designating Friday, December 23, 2016 as the floating holiday.
II. LEASE AGREEMENT - 1000 VICTORY DRIVE:
Approval is requested to enter into a lease agreement with the Disabled
American Veterans (DAV) to lease the building at 1000 Victory Drive for $1 per
year for a period of five years. The tenant will be responsible for all
maintenance, utilities and liability insurance. The property at 1000 Victory
Drive has been leased for several years by the Chattahoochee Valley Veterans
Council (CVVC) as a Veterans Welcome Center for $1 per year but the CVCC has
decided not to renew the lease that expires on December 31, 2015.
III. QUITCLAIM DEED FROM UPTOWN TO CITY OF COLUMBUS:
Approval is requested to accept a Quitclaim Deed recorded at deed book
10936, Page 336 to add the proper legal description to the Eagle & Phenix Power
Houses and the Access Easement. The property is located around the power house
structures and along the riverbed utilized as a temporary access road that
leads to the power houses per the attached plats.
IV. ENTERPRISE ZONE: ANKERPAK, LLC - 3961 CUSSETA ROAD:
Approval is requested for Ankerpak, LLC to receive enterprise zone tax
abatements at their new facility located at 3961 Cusseta Road. This project
will retain 65 jobs and create an additional 15 positions over a ten year
period. Ankerpak has met the necessary requirements set forth in ordinance
98-30 to receive a 10 year abatement on property taxes as part of the Columbus
Business Development Center.

V. ENTERPRISE ZONE: BD&K GOODS, INC. - 1333 CUSSETA ROAD:
Approval is requested for BD&K Foods, Inc. to receive enterprise zone
tax abatements at their new facility located at 1333 Cusseta Road. The
addition will allow the company to extend the warehousing and distribution
components of the business which will result in 35 new jobs and an initial
investment of 1.7 million dollars. BD&K Foods has met the necessary
requirements set forth in ordinance 98-30 to receive a 10 year abatement on
property taxes as part of the Columbus Business Development Center.
VI. FY16 LOCAL GOVERNMENT & IMPROVEMENT GRANT (LMIG) FORREST ROAD WIDENING
PROJECT:
Approval is requested to accept a FY16 Local Government & Improvement
Grant from the Georgia Department of Transportation for the Forrest Road
Widening Project Phase I. The LMIG Agreement will provide approximately $1.5
million to cover the added cost for construction from the original budget.
VII. 2015-2016 VICTIMS OF CRIME ACT (VOCA) Continuation Grant:
Approval is requested to authorize the District Attorney/Victim Witness
Assistance Program to submit and if approved, accept a grant for $84,138 from
the Georgia Criminal Justice Coordinating Council along with $21,035 from the
5% Crime Victim Surcharge Fund allocated in the FY16 Budget to provide funding
for the Crime Victim Assistance in the in the Chattahoochee Judicial Circuit
from October 1, 2015 to September 30, 2016. Along with an additional In-Kind
Match of $5,328 to be contributed by volunteer hours. It is requested that the
multi-governmental fund be amended by the amount awarded plus the local match.
VIII. 2015-2016 VICTIMS OF CRIME ACT (VOCA) SPECIAL AWARD THROUGH PROSECUTING
ATTORNEY'S COUNCIL (PAC):
Approval is requested to authorize the District Attorney?s
Victim-Witness Assistance Program to submit and if approved, accept a grant for
$81,836 from the Prosecuting Attorney?s Council along with $15,344 from the 5%
Crime Victim Surcharge Fund allocated in the FY16 Budget to provide funding for
Crime Victim Assistance in the Chattahoochee Judicial Circuit from October 1,
2015 to September 30, 2016. Along with an additional In-Kind Match of $5,114 to
be contributed by volunteer hours. It is requested that the multi-governmental
fund be amended by the amount awarded plus the local match.

Scope of RFP
It is the intent of Columbus Consolidated Government (the City) to enter into
an annual contract with qualified offeror(s) to provide Consulting and
Insurance Broker Services (the Broker) to represent the City in various
insurance markets.
The initial term of the contract will be for one (1) year with the City?s
option to renew for four (4) additional twelve-month periods.

Columbus Consolidated Government is seeking proposals from waste haulers to
provide waste collection services on an as-needed basis for Columbus, Muscogee
County, Georgia.
