I. CCG EMPLOYEE HEALTH & WELLNESS FACILITY LEASE AGREEMENT:
Authorization is requested to enter into a lease agreement with new owner of
Physicians Realty Trust for specified medical/office space at 2000 Tenth
Avenue, Suite 410, for the Employee Health/Wellness Center. An agenda report

II. STREET ACCEPTANCE - LALLAROOK DRIVE, THAT PORTION OF FRANK HOUSER AVENUE,
THAT PORTION OF SHOWTIME DRIVE AND THAT PORTION OF CONFETTI BLUSH DRIVE LOCATED
IN OLD TOWN MIXED USE COMMUNITY - REPLAT OF LOT 4A:
Authorization is requested for the acceptance of a deed to Lallarook
Drive, that portion of Frank Houser Avenue, that portion of Showtime Drive and
that portion of Confetti Blush Drive located in Old Town Mixed Use Community.
The Department of Engineering has inspected said streets and recommends the

III. ACCEPTANCE OF DETENTION BASIN - THIRD ADDITION TO WHITESVILLE WALK:
Authorization is requested for the acceptance of the deed to the
Detention Basin located in Third Addition to Whitesville Walk. The owner has
maintained basin since construction for the period of at least two years as

IV. CANINE PROGRAM:
Authorization is requested to enter into an Intergovernmental Agreement
with the Bureau of Alcohol Tobacco Firearms and Explosives (BATFE) to enable
the Columbus Department of Fire/EMS to host the Accelerant Dog Canine Program
and to purchase a Medium SUV for program, if approved. An agenda report,

Scope of RFB
Provide preventive maintenance and/or repair service for pottery kiln at
various locations. (See Attachment A for locations) The contract period will
be for one (1) year with the option to renew for four (4) additional
twelve-month periods.

Scope of RFB
Provide alternators and starters to METRA Transit System and the City Fleet
