I. ALLOCATION OF COMMUNITY DEVELOPMENT BLOCK GRANT FUNDS TO THE BOOKER T.

WASHINGTON REDEVELOPMENT PROJECT:



Authorization is requested to allocate up to $2,500,000 in Community

Development Block Grant (CDBG) funds for the redevelopment of the Booker T.

Washington Apartments (BTW) located at 500 5th Avenue. The CDBG funds would be

used towards the demolition and site work activities. An agenda report and

resolution are attached.





II. STREET ACCEPTANCE - GALLANT DRIVE, THAT PORTION OF CAPOT DRIVE, PORTION OF

ADMIRAL DRIVE LOCATION IN SECTION ONE, MIDLAND DOWNS:



Authorization is requested for the acceptance of a deed to Gallant

Drive, that portion of Capot Drive and that portion of Admiral Drive location

in Section One, Midland Downs. The Engineering Department has inspected said

street and recommends the acceptance. An agenda report, map and resolution are

attached.





III. LEARN TO SWIM GRANT THROUGH AFLAC FOUNDATION:



Authorization is requested to submit an application and, if approved,

accept a grant in the amount of $7,848.00 for the first year of award and later

amounts awarded in year two and three of the pilot program from the AFLAC

Foundation through the Community Foundation of the Chattahoochee Valley. An

agenda report and resolution are attached.





IV. ASSOCIATION OF COUNTY COMMISSIONERS OF GEORGIA CIVIC AFFAIRS FOUNDATION

GCIP GRANT:



Authorization is requested to submit an application and, if approved,

accept $2,173.00 or as otherwise awarded from the Association of County

Commissioners of Georgia Civic Affairs Foundation to hire an intern to update

forms used in the Records Management System for a ten week period of the Fall

of 2015, with no local match requirement, and to amend the Multi-Governmental

Fund by the amount of the award. An agenda report and resolution are attached.

1. Consulting and Insurance Broker Services for Property and Crime Insurance

(Annual Contract) - RFP No. 15-0029

Scope of RFP

It is the intent of Columbus Consolidated Government (the City) to enter into

an annual contract with qualified offeror(s) to provide Consulting and

Insurance Broker Services (the Broker) to represent the City in various

insurance markets.



The contract term will be for one year with the City?s option to renew for four

(4) additional twelve-month periods.



June 3, 2015

1. Cameras - RFB No. 15-0028

Scope of RFB

The Columbus Consolidated Government (the City) is seeking vendors to provide

the following cameras to be used by Officers of the Columbus Police Department:

a. Body Worn Cameras ? Quantity of fifty (50). These cameras will be used by

individual Officers.

b. Point and Shoot Cameras ? Quantity of two-hundred-fifty (250). These

cameras will be used to record photographs related to domestic violence

investigations and other circumstances where photographic evidence is required.

2. Transit Enhancements (Annual Contract) - RFB No. 15-0030

Scope of RFB

METRA is requesting qualified vendors to provide the following Transit

Enhancements: Mesh Trash Receptacles, 8? Perforated Steel Benches with backs

and without backs, Jumbo Horizontal Message Centers and Single Face Info Center

to METRA Transit System on an ?as needed? basis. The contract term will be for

one (1) year with the option to renew for four (4) additional twelve-month

periods.

3. Landscape Materials (Annual Contract) - RFB No. 15-0031

Scope of RFB

Provide landscape material consisting of pine straw, wheat straw and

hay on an ?as needed? basis to the Columbus Consolidated Government (the City)

to be utilized by Park Services, Public Works and Cooperative Extension

Departments. The contract term will be for two years with the option to renew

for three additional twelve month periods.

June 5, 2015

1. Thermal Imagers (Annual Contract) - RFP No. 15-0027

Scope of RFP

The Columbus Consolidated Government is requesting proposals to provide thermal

imagers to be used by the Fire and EMS Department. The thermal imagers will

not be purchased at this time, but the department wishes to establish a

contract for future purchases.



The contract term will be for one year with the City?s option to renew for four

(4) additional twelve-month periods.



June 12, 2015

1. Treatment Providers for Mental Health Court and Veterans Treatment Court

(Annual Contract) - RFP No. 15-0032

Scope of RFP

The Columbus Consolidated Government, Mental Health Court and Veterans

Treatment Court, is seeking proposals from qualified firms to provide treatment

to participants in the following areas: PTSD Counseling and Psychological

Assessment. The term of the contract will be for two (2) years with the option

to renew for three (3) additional twelve-month periods.



June 17, 2015

1. Bus Shelters (Annual Contract) - RFB No. 15-0026

Scope of RFB

METRA is requesting qualified vendors to provide Bus Shelters on an ?as needed?

basis. The contract term will be for two years with the option to renew for

three additional twelve-month periods.



2. Trailhead Buildings for South Lumpkin Multiuse Trail ? RFB No. 15-0032

Scope of RFB

Construct the Torch Hill Road trailhead building and the Cusseta Road trailhead

building for the South Lumpkin Multiuse Trail.

Scope of RFP

Columbus Consolidated Government is requesting proposals for a qualified vendor

to provide all necessary equipment, materials, transportation, and labor for

residential and commercial demolition services.
