I. PRE-CONSTRUCTION FOR ROAD IMPROVEMENTS:



Authorization is requested to proceed with executing and funding the

pre-construction requirements for Road Improvement Projects funded through the

Local Option Sales Tax and Special Purpose Local Option Sales Tax to include

Right of Way Acquisition, Permanent and Temporary Construction Easement,

Utility Relocation, Environmental Mitigation and other necessary agreements to

prepare projects for construction. An agenda report and resolution are

II. BENNING HILLS TANK EASEMENT:



Authorization is requested to grant an easement to the Columbus Water Works on

property located at Benning Hills ? Land Lot, 15 7th District in Columbus,

Muscogee County, Georgia. The easement requested is depicted on the

attachments with the easement area highlighted in yellow. An agenda report,

III. GEORGIA DEPARTMENT OF TRANSPORTATION - SAFE ROUTES TO SCHOOL GRANT -

SIDEWALKS NEAR CLUBVIEW ELEMENTARY SCHOOL:



Authorization is requested to submit an application and if approved accept

$164,131.00 or the amount awarded from the Georgia Department of Transportation

- Safe Routes to School Grant Program to have sidewalks added to segments of

six streets near Clubview Elementary School on the south side of the campus.

IV. GEORGIA DEPARTMENT OF NATURAL RESOURCES - GEORGIA RECREATIONAL TRAILS GRANT

PROGRAM:



Authorization is requested to submit an application and if approved accept

$120,000.00 in grant funds or the amount awarded from the Georgia Department of

Natural Resources for the Columbus Handicapped Trail improvement at Cooper

V. Unused Sick Leave Payment:



Authorization is requested to approve payment to employees for unused

sick leave in accordance with 16B-15-6(2) of the Columbus Code of Ordinances.

VII. CITY OF COLUMBUS 10-YEAR PLAN TO END HOMELESSNESS - UPDATE:



There will be an update on the City of Columbus 10-Year Plan to End

Homelessness by the Cunningham Center for Leadership Development. (Postponed

for November 30 Work Session Agenda)





VIII. BENEFIT MANAGER PRESENTATION:



Director Tom Barron, Human Resources, will provide an update on health

care benefits.

1. Ready Mix Cement (Annual Contract) ? RFB No. 11-0026

Scope of Bid

Provide approximately one hundred cubic yards of ready mix cement on an ?as

needed basis? to be used by the Public Services Department of the Columbus

Consolidated Government. The contract period will be for one (1) year with the

option to renew for four (4) additional twelve-month periods.



2. Cargo Drive and Transport Boulevard Roundabout ? RFB No. 11-0028

Scope of Bid

Construct a roundabout at the intersection of Cargo Drive and Transport

Boulevard, all in accordance with the designated plans and specifications. The

contractor shall be required to maintain public access to all residences

located within the project area and normal traffic.



3. Metra Bus Shop Roof Replacement ? RFB No. 11-0024

Scope of Bid

Provide all labor, equipment and materials to re-roof the METRA Bus Shop. The

work includes removing and disposing of the existing SPUF/BUR roofing,

installing a new TPO roof system with new insulation, new metal flashing and

coping, and replacing existing skylights.

Provide and maintain copier services to various departments of the Columbus

Consolidated Government. The contract term will be for two years, with the

Scope of RFP

The Columbus Consolidated Government seeks qualified firms or individuals to

operate the Pro Shop located in the new ice skating facility. Operation is
