I. TALBOTTON ROAD - WARMS SPRING ROAD WIDENING PROJECT - GEORGIA DEPARTMENT OF

TRANSPORTATION:



Authorization is requested to execute an Option for Right-of-Way Agreement with

the Georgia Department of Transportation for the quit claiming of city

property located at 1708 Warm Springs Road for the Talbotton Road/Warms Spring

Road Widening Project. An agenda report, agreement and resolution are

II. WILLIAMS ROAD -- LIMITED ACCESS BREAK:



Authorization is requested for the relocation of the existing break in Limited

Access along Williams Road, between Fortson Road and Veterans Parkway, to

provide access to the development for east bound traffic on Williams Road. An

III. FY12 OTHER LOCAL OPTION SALES TAX REALLOCATION - SHERIFF'S OFFICE:



Authorization is requested to reallocate $18,000 of the FY12 Budget-Other Local

Option Sales Tax from the Jail Needs Assessment to the Sheriff's Office

Training Facility-Telephone Installation ($12,000), Metal Detector replacement

for East Wing ($4,000) and ID Card Printer replacement ($2,000). An agenda

V. 2011 VICTIMS OF CRIME ACT (VOCA) - GEORGIA CRIMINAL JUSTICE COUNCIL - GRANT:



Authorization is requested to submit an application and, if approved, accept a

grant for $84,838 from the Georgia Criminal Justice Coordinating Council along

with $16,968 from the Five Percent Crime Victim Surcharge Fund allocated in the

FY12 Budget to provide funding for Crime Victim Assistance in the Chattahoochee

Judicial Circuit from October 1, 2011 to September 30, 2012. An agenda report

Emergency Purchase ? Information Only



Emergency Replacement of Security Gate at Muscogee County Prison:



On August 29, 2011, Warden Adamson, Muscogee County Prison, requested an

emergency purchase to replace a security gate that was damaged by a Public

Services vehicle. The 12? high x 14? wide gate normally ?latches? at the top

with only 1 inch of clearance between the gate pole and the stationary pole.

The damage to the gate caused about a one-foot opening at the top of the gate

between the two poles, allowing enough space for a prisoner to escape.

Additionally, the gate is an electric sliding gate on wheels requiring

hydraulic power to open and close the gate. The hydraulic operator is

inoperable and needs replacing and the hard plastic wheels on the gate are

cracking, as well. The gate is located in the area where prisoners return

from work detail daily before being led back into the building.



Muscogee County Prison staff obtained the following prices quotes for the gate

repair:

Denny Enterprises (Columbus Georgia) $10,041.00

Lowery Fence (Waverly Hall, Georgia) $13,481.00

Georgia Fence Wholesale (Columbus, Georgia) $14,000.00



The City Manager approved the emergency gate replacement, on August 29, 2011,

for the quoted amount of $10,041.00 from Denny Enterprises.

Scope of Bid

Replace fencing around two (2) baseball fields at Britt David Park, which is

located at 2700 West Britt David Road, Columbus, GA 31909.



2. Dial-A-Ride Buses ? RFB No. 12-0010

Scope of Bid

Provide two (2) Dial-A-Ride para-transit buses to be used by the METRA

Department of Transportation. The buses will be 26-foot low floor medium duty

buses. The buses will be used by METRA to provide ADA Complementary

Paratransit Service to eligible persons with disabilities who are, because of

their disability, unable to board, ride or disembark from an accessible vehicle

in METRA's regular bus service. Service is provided with special lift-equipped

buses on a curb-to-curb, shared ride basis. The vehicles are replacement units.

1. Household Hazardous Waste Sorting & Disposal Services (Annual

Contract) ? RFP No. 12-0004

Scope of RFP

Provide household hazardous waste sorting and disposal services for two events

per year. The service allows the citizens of Columbus, Georgia the opportunity

to discard household hazardous waste. The contract term shall be for three

years, with the option to renew for two (2) additional twelve-month periods.

Renewal will be contingent upon the mutual agreement of the City and the

Contractor.

1. Back Support Safety Belts & Safety Vests (Annual Contract) ? RFB No. 12-0006

Scope of Bid

Provide approximately 200 back support safety belts and approximately 5,750

safety vests and approximately 2,500 safety t-shirts to Columbus Consolidated

Government (the City) on an "as needed" basis. The contract term will be for
