ACTION Program.

information to her. Will do a follow-up.

Called Mr. Castleberry at Ft. Benning to schedule Color Guard for Disability?s

Awareness Day in May. Left a message for Mr. Castleberry.

Received call from Mr. Walton with a local Lodge Non-Profit. Requesting

information on volunteering and sponsorship for Mayor?s Committee. 706-464-8102 Returned

call to Mr. Walton and invited him to Committee meeting and gave him

information on volunteering and different projects.

Received call from Eva Howard. Requesting information on getting handicap

signage. 706-327-2845 Talked to Ron Hamlett in Engineering for information. Left message

for Ms. Howard with information to contact Citizen Services or local sign

company.

Received call from Darlene Middleton from (CWIC) Work Coordinator. She

requested a meeting to talk about her services and information on upcoming

meeting. 706-566-5671 Returned call and left message with meeting information.

Received call from Ms. Rigdon concerning building a ramp for her home.

Received call from Ms. Lowman. Requesting information for Rent assistance. Returned call to

Ms. Lowman and gave her community resources for assistance.

Received call from Annette Gibney from Parks/Recs. Responding to my request

for more information on local handicap accessible pools. Ms. Gibney gave me

information to pass on to citizens.

Received call from Art Thomason (Civic Center); inquiring of opportunities for

Health/Fitness instruction to work with young kids/adults with disabilities. Informed Mr.

Thomason of the Parks/Recs Therapuetic Program he can refer her to.

Received call from Lillian Childs requesting assistance to have ramp built at

home. 706-687-7253 Took Ms. Childs address and contact information to forward to Connie

Freier and CATOC for their ramp project.

Ramp Committee assistance.

Received call from Donte Pittman. 229-254-7343

Received a call from Alicia Brown 706-393-6739 requesting information on

getting a proclamation and assistance with fashion show her disabled cousin is

hosting. Forwarded Ms. Brown information on sponsorship letters and contact information

for proclamation request.

Received call from Darlene Middleton from Dept. of Labor. Called and left another

message for Ms. Middleton.

Received call from Ms. Brown 706-536-7669 requesting information on

transportation for appointments, etc. Ensured Ms. Brown?s knowledge of Metra and

other agencies in Columbus that provide non-emergency transportation.

Received call from Mr. Castleberry from Community Affairs on Ft. Benning. He

informed me they would not be available on May 21 for color guard. He passed on

contact information for local reserve unit. 468th Maneuver Enhancement Brigide

National Guard, 706-685-7588/7505 or 404-578-9208. Updated the Committee and was referred to

request color guard from Muscogee County School District. Col. Roy Plummer. I

have left messages for him to contact me.

Received call from Patricia Crawford. 706-682-9063

Received call from Mary Hewey requesting information on Community

resources/training for her. (Recently loss most her vision and needs training

in attending to daily basic needs). Forwarded Ms. Hewey?s contact information to Connie

Freier of Tools for Life and Susan Knight of National Association for the

Blind for more resources and assistance.

Received call from Darlene Middleton from Dept. of Labor. Gave Ms. Middleton

information on our next events and talked to her about her services and how we

can work together.

Received call from Jeanette Brown 706-563-8878. Requesting information on

getting a ramp built. I gave Ms. Brown information for Project Care and took her

contact information to pass on to CATOC and Connie Freier.

Received correspondence from Committee members on upcoming events. Topics:

Rescheduling of Disabilities A. Day, scheduling/planning Long Road Home event,

information for Disabilities Awareness Day It was determined that those receiving

awards must be a Columbus Resident.

Follow-up with Alicia Brown. Faxed sponsor letter and requested more information on

Fashion Show for disabled

Received call from Ms. Deanna Brown requesting information on repair for deck.

Returned call to Donte Pittman. 229-254-7343 Left message.

Follow-up with Mike/Barbara Robinson. (Wanted information for advocacy

assistance.)

706-323-6260. (His wife was terminated from school and he wanted to know her

rights.) Gave Mr. Robinson information for GA Advocacy Office and ADA to contact for

assistance.

Follow-up with Patricia Crawford. (Information on State Laws) Left message.

Received follow-up information from Susan Knight. She contacted Ms. Hewey and

will be doing a home visit. (She recently loss vision, and needs assistance

with resources/adapting.)

Received email from Crystal Ward requesting information on local agencies and

support groups for Autism and other disabilities. Also on the Mayor?s

Committee?requested to have brochure mailed. Gave Ms. Ward local agency contact

information. (Autism Hope Center and Mays Institute). Mailed brochure also.
