I. Call to order
Dr. Wise called the Columbus Board of Health to order at 1:00pm.

II. Approval of Minutes
Dr. Wise asked if there were any questions or comments on the previous minutes
received (August 26, 2015).
Dr. Wise asked for a motion to approve the minutes. A motion was made by Ms.
Cathright, seconded by Dr. Chhokar and approved by all members present.

III. District Health Director?s Report
Dr. Wise called on Dr. Townsend to give the Director?s Report;
1. Introduced the new PIO ? Pamela Kirkland to the board
2. Last month we completed 65 sports physicals for students at Spencer High
School.
3. We are preparing for ICD10 conversion; working with Mitchell & McCormick

IV. Financial Report Dr. Wise called on Joanne Strickland to present the financial
report to the Board. Ms. Strickland presented the following;
1. Written copy of the financial report and overview for FY16 budget is
$5,677,814 with one revision.
2. Expenses were $839,718 which is $79,747 more that the same time last year,
which is under target for 2 months of operations.
3. Fees collected are $230,644 which is up from same time last year by $5,134.
4. All other programs on target for 2 months of operation.

V. Program Reports Dr. Wise asked if there were any Program reports.
Sheila Mayfield, County Nurse Manager
1. Provided data on clients from July 1 ? August 31;
a. Total clients 7878
b. Initial visits to HD 90% - return visits 10%
2. Staff from WCHD received a state award for partnership with The University
of GA Extension Association of Family and Consumer Sciences for their outreach
in providing education and contraceptive services for the undocumented
residents of Columbus
3. Colethia Moore was selected to receive the Walt Orenstein award during the
2015 Annual GA Immunization Conference. The award recognized Mrs. Moore for her
dedication and innovation in education and providing immunization services.
4. upcoming in the next few months;
a) Scoliosis check
b) Flu Shots outreach (schools, private businesses, etc.)
c) Breast Cancer Awareness
Recognition of National Immunization Awareness Month
Immunization Audits
Working on partnership with Social Security Office, DMV, and Post Office

5. Provided Environmental Health numbers from August 16 to September 15 which
included number of permitted establishments and routine inspections.
6. Changes to the Food Service Program ? provided handout
