CITY MANAGER'S AGENDA

I. HEALTH AND WELLNESS CENTER CONTRACT:



Approval is requested to renew the annual contract for the Health and

Wellness Center vendor, Transform HealthRx of Georgia for a twelve month

period beginning January 14, 2016 and ending January 13, 2017. Transform

HealthRx will furnish all labor, materials and equipment required and meet all

other requirements or conditions imposed in accordance with the City's RFP

dated November 16, 2011 and the Contractor's agreement.





II. ACQUISITION OF PROPERTY 6250 WARM SPRINGS ROAD:



Approval is requested to acquire the property located at 6250 Warm

Springs Road for a sum of $10, plus $5,300 for appraisal fees incurred by the

State of Georgia to determine the fair market value and legal fees associated

with the transfer. Acquisition of this property will allow the City to expand

Flat Rock Park and the City will not be required to relocate the driveway into

Flat Rock Park should the property be sold to a private party.





III. MILITARY GENERATOR ACCEPTANCE:



Approval is requested to enter into an Inter-Governmental Agreement

with the Georgia Forestry Commission to enable the Columbus Department of

Fire/EMS to accept two (2) military grade 15 KW portable generators to be

utilized for emergency operations. There is a $100 transfer fee for each

generator for a total cost of $200 and the funds are available in the Fire and

EMS FY16 Budget.





IV. 2015-2016 VOCA CONTINUATION GRANT:



Approval is requested to submit an application and if approved, accept

a new additional VOCA grant of $49,444 from the Prosecuting Attorneys' Council

of Georgia, to provide additional funding for a Crime Victim Advocate in the

office of the Solicitor General for Muscogee County from October 1, 2015

through September 30, 2016, with a $12,372 in-kind match, and amend the

multi-governmental fund by like amount.

Columbus Consolidated Government is requesting bids to provide all labor,

equipment and materials to remove and dispose of two (2) existing gym package

units, and replace with two (2) gas package units at the Carver Park Recreation

Center Gym.

Scope of Bid

The Columbus Consolidated Government wishes to purchase four (4) Chevrolet

Tahoe 2WD Police Pursuit Vehicles to be used by the Columbus Police Department.

Scope of Bid

The Columbus Consolidated Government wishes to purchase two (2) 7 YD Dump

Trucks to be used by the staff of the Rainwater Management Division.

the Columbus Landfill and Other City Locations (Annual Contract) ? RFB No.

Scope of Bid

Contractor pays a percentage to the City for the removal of metal, bulky items

and similar material from the Landfill and other City locations.

1. Waste Collection Services (Annual Contract)

Scope of RFP

Columbus Consolidated Government is seeking proposals from waste haulers to

provide waste collection services on an as-needed basis for Columbus, Muscogee

County, Georgia.



The initial term of the contract will be for five (5) years with the City?s

option to renew for five (5) additional twelve-month periods.
