I. RETIREE HEALTH INSURANCE PLAN:



Approval is requested authorizing the renewal of the Medicare eligible

healthcare plan benefits and the United Healthcare Medicare Advantage plan. The

renewal rate will increase by 3.9% increasing the City's contribution for

CY2018. The Retiree Benefits Committee, ShawHankins Benefits Consulting and

the Human Resources Department recommends accepting the renewal with United

Healthcare.





II. ALLOCATION OF CDBG FUNDING TO THE COLUMBUS AREA HABITAT FOR HUMANITY FOR

THE CONTINUING HOPE PROGRAM:



Approval is requested to enter into a CDBG Written Agreement, between

the Columbus Consolidated Government and the Columbus Area Habitat for Humanity

to award Community Development Block Grant (CDBG) funds in the amount of

$149,829 to administer the Continuing Hope Program to assist the

elderly/disabled homeowners with repairs to their homes. The Continuing Hope

Program will be based on an application list for senior/elderly homeowners

(62+) who are low-income (80% or below AMI). The program will provide the

homeowner the required repairs to their homes such as roofs, HVACs, plumbing,

and electrical with an amount up to $10,000 per house. Through the

partnership, the Columbus Area Habitat for Humanity will assist approximately

13 elderly/disabled homeowners.





III. ALLOCATION OF CDBG FUNDING TO ACCESS 2 INDEPENDENCE A HOME ACCESS PROGRAM:



Approval is requested to enter into a CDBG Written Agreement, between

the Columbus Consolidated Government and Access 2 Independence to award

Community Development Block Grant (CDBG) funds in the amount of $80,000 to

administer Home Access Program to assist disabled persons with accessibility

modifications to their homes. The Home Access Program will be based on a

referral list for disabled residents who are low-income (80% or below AMI).

The program will provide the resident with the required improvements to their

homes such as ramps, grab bars the widening of doorways, and the modifications

to bathtubs/showers with an amount up to $5,000 per house. Through this

partnership, Access 2 Independence will assist approximately 14 disabled

residents.

IV. ALLOCATION OF CDBG FUNDING TO TRAINING INNOVATIONS FOR A SECTION 3

EMPLOYMENT TRAINING PROGRAM:



Approval is requested to enter into a CDBG Written Agreement, between

the Columbus Consolidated Government and Training Innovations to award

Community Development Block Grant (CDBG) funds in the amount of $25,000 to

administer a Section 3 Employment Training Program. The Section 3 Employment

Training Program will target citizens who are low-income (80% or below AMI).

The program will provide training and employment opportunities by working with

low-income residents and connecting them with local contractors, etc. to assist

them with finding jobs.





V. STREET ACCEPTANCE - CAPOT COURT, THAT PORTION OF CAPOT DRIVE AND THAT

PORTION OF ADMIRAL DRIVE LOCATED IN SECTION TWO, MIDLAND DOWNS:



Approval is requested for the acceptance of Capot Court, that portion

of Capot Drive and that portion of Admiral Drive located in Section Two,

Midland Downs. No City funds are involved until maintenance is assumed after

the two-year warranty. The Engineering Department has inspected said street

and recommends acceptance of same.





VI. USE OF OLOST PUBLIC SAFETY CONTINGENCY:



Approval is requested authorizing the use of Other Local Option Sales

Tax (OLOST) in the FY18 budget to provide for the installation of a mobile

kitchen due to repairs of the kitchen floor at the Muscogee County Jail at an

estimated cost of $109,145 - $164,945 as well as to purchase and/or replacement

of inoperable/obsolete tasers and equipment at a cost of $20,164.96 for the

Sheriff?s Office.





VII. ISSUANCE OF A TAX ANTICIPATION NOTE IN FY2018:



Approval is authorizing the solicitation of bids to obtain a Tax

Anticipation Note (?TAN?) in an amount not to exceed $9,200,000 and to pay

certain fees associated with the issuance.

VIII. DONATIONS FOR VETERAN'S MEMORIAL COLUMBARIUM: (Add-On)



Approval is requested to accept cash donations to be used for the

construction and maintenance of the columbarium and for urns and other

paraphernalia for interment of our indigent veterans ashes. Such funds will be

placed in a special fund and utilized solely for the upkeep of the columbarium,

urns, landscaping, flags and other items.





IX. ACCEPTANCE OF DONATIONS IN SUPPORT OF "THE DREAM LIVES," DR. MARTIN LUTHER

KING, JR. (MLK) COMMEMORATION EVENT:



Approval is requested to accept approximately $23,000 from FY2017 and $50,000

for FY2018 in support of the MLK commemorative event and to expend monies to

cover said expenses of the event approved by the Commission and the Mayor, but

in no event in excess of amounts received in donation. Any donated amounts in

excess of the costs of the MLK commemorative event will be retained in a

designated account of the Commission to be used for similar events sponsored by

the Commission in future years.

1. CS 2228/Ft. Benning Road from CS 325/Levy Road to Fort Benning ? RFB No.

08-0011

Scope of Bid

Construct the proposed road and landscape improvements along Ft. Benning Road

between Shelby Avenue and the entrance to Fort Benning in Columbus, GA. The

work primarily involves installation of sidewalk, street pavement, concrete

curb, and landscaping as depicted on the drawings provided as part of these

Contract Documents. The DBE goal for this project is 14%. The Georgia

Department of Transportation Standard Specifications, 2013 Edition, GDOT

Supplemental Specifications Book, 2016 Edition, and applicable Supplemental

Specifications and Special Provisions apply to this project.



Bidders are required to be pre-qualified or registered subcontractors with

Georgia Department of Transportation (GDOT); Bidders submitting bids in excess

of $2,000,000.00 must be prequalified with GDOT; Subcontractors shall be

pre-qualified or registered with GDOT.

Located Throughout Columbus, Georgia (Annual Contract); RFP No. 18-0009

Scope of RFP

Install or replace sidewalks, ADA ramps and minor concrete work, such as

driveway aprons, concrete islands, curb and gutter, etc., throughout

Columbus-Muscogee County, on an as-needed basis. The term of the contract

shall be for two years, with the option to renew for three (3) additional

twelve-month periods.
