I. PROPERTY EXCHANGE AGREEMENT WITH COLUMBUS REGIONAL MEDICAL CENTER:



Authorization is requested to execute a Property Exchange Agreement and

Addendum to Medical Office Lease Agreement for right-of-way at the corner of

Talbotton Road and Hamilton Road for parking and entrance feature improvements,

for the abandonment of an alleyway connecting Hamilton Road and 7th Avenue to

Columbus Regional Medical Center and to exchange rent for the Employee Health

and Wellness Center for the vaule of the property. An agenda report, lease

II. QUIT CLAIM DEED ON PROPERTY LOCATED AT 5117 12TH AVENUE:



Authorization is requested to execute a Quit Claim Deed to release the

City's interest in the property located at 5117 - 12th Avenue purchased with

CDBG funds for Affordable Housing. Columbus Housing Initiative, Inc.,

developed an affordable home and sold to an income qualified buyer. An agenda

III. FALL LINE TRACE EASEMENT - CALVARY FREE WILL BAPTIST CHURCH:



Authorization is requested to enter into an Access, Parking and

Landscape Easement agreement with Calvary Free Will Baptist Church. Calvary

Free Will Baptist Church is requesting an access point from their property to

the Fall Line Trace that will begin at their parking lot and run to the edge of

the Trace. The Church will be responsible for all improvements. An agenda

IV. 14TH STREET PEDESTRIAN BRIDGE SUPPLEMENTAL AGREEMENT - GEORGIA DEPARTMENT

OF TRANSPORTATION:



Authorization is requested to enter into a supplemental agreement with

the Georgia Department of Transportation (GDOT) for $76,178 in additional funds

with a local match of $19,044 or 20% for a total of $95,2221.90 and amend the

contract for the 14th Street Pedestrian Bridge (P.I.0009401), with Scott

Bridge, Inc., for the redesign of pier construction along with changes to

bridge deck overhangs, bridge railing, lighting, etc. An agenda report and

V. ACQUISITION OF PROPERTY - 525 TENTH AVENUE:



Authorization is requested to purchase .98+/- acres of property at 525

Tenth Avenue to develop a storage facility for the Columbus Police Department.

VI. FIREHOUSE SUBS PUBLIC SAFETY FOUNDATION GRANT APPLICATION:



Authorization is requested to submit an application, and if approved,

accept a small grant for $20,000 from the Firehouse Subs Public Foundation for

the Muscogee County Marshal?s Office to enhance the Muscogee County Training

VII. STREET ACCEPTANCE - OLD GUARD ROAD AND EXTENSION OF AMERICAN WAY:



Authorization is requested for the acceptance of a deed to Old Guard

Road and the extension of American Way located in part of Land Lots 264 and

269, 19th District and to accept an easement where a portion of the guard

rails, which were erected as a condition of the street acceptance, are located

on private property. The Department of Engineering has inspected this street

VIII. STREET ACCEPTANCE - CYPRESS RIDGE DRIVE LOCATED IN CYPRESS RIDGE

SUBDIVISION:



Authorization is requested for the acceptance of a deed to Cypress Ridge Drive

located in Cypress Ridge Subdivision. The Department of Engineering has

inspected this street and recommends the acceptance. An agenda report, map and

Emergency Purchase ? Information Only



Emergency Purchase of Data Recovery Services:



On March 6, 2013, the Finance Director requested an emergency purchase for data

recovery services for the Tyler Eagle Real Estate Imaging System used by the

Clerk of Superior Court.



The disk drives of the Dell server, which houses the Eagle software and Real

Estate Images, failed. This failure of the server cut off access to property

records needed to close on real estate transactions. While data is being

recovered, an alternative way to access the data has been approved by Judge

McBride. City staff including the Mayor, Judge McBride, City Manager, Superior

Court Clerk, IT and Tyler have been working diligently to address the issues

and get the system back up and running. An exact time cannot be given as to

when everything will be up and running, but everything is being done to

expedite that process in every way possible.



The Real Estate Imaging software vendor, Tyler Technologies, recommended a

company out of Atlanta (headquartered in Los Angeles, CA), Secure Data Recovery

Services, to perform an emergency data recovery effort on the lost images.

Secure Data Recovery Services has submitted an invoice totaling $19,420.80 for

their services. Payment will be wired to the company when the restored images

have been confirmed by Tyler Technologies. Tyler Technologies will also

invoice for recovery services at $140 per hour. An estimate of this cost has

yet to be determined. The total project is estimated at $25,000.



The City Manager approved the emergency on March 6, 2013.



Funds are budgeted in the FY13 Budget: General Fund ? Miscellaneous ?

Contingency; 0101-590 - 2000 - CONT ? 9801.

Scope of RFP

Soliciting proposals from qualified vendors to provide exclusive soft drink

locations. The term of the contract will be for ten (10) years.

Provide comprehensive medical services for inmates at the Muscogee County Jail

1. Oakland Park Storm Drainage Improvements Phase I ? RFB No. 13-0047

Scope of Bid

This work shall consist of providing storm drainage improvements in the Howe

Avenue area (Sub-Basin ?C?) and the South Plant Road area (Sub-Basin ?M?) of

Oakland Park. The Contractor shall be required to maintain public access to

all residences located within the project area and maintain normal traffic.

1. Statement of Qualifications for Professional Design Services for the

Renovation and Additions to the Muscogee County Jail ? RFP No. 13-0025

Scope of RFP

Columbus Consolidated Government (the Owner) requests firms to submit

statements of qualifications for professional design services for the

renovation and additions to the Muscogee County Jail, which is located at 700

10th Street in Columbus, GA.



It should be noted that the Owner desires to procure the services from a team

comprised of an architectural firm that specializes in developing jail/prison

projects, who will serve as the lead architect, and a local architect.

1. Right-of-Way Digital Imaging and Extraction of Pavement Condition Data ? RFP

No. 13-0026

Scope of RFP

Columbus Consolidated Government (CCG) requests proposals from qualified firms,

with proven experience, to collect ground-level right-of-way digital images of

the roadway network. The images shall be used to update the pavement condition

ratings stored in CCG?s proprietary pavement management system. Under the terms

of this RFP, the successful respondent is to work with CCG?s pavement

management consultant to deliver the pavement condition ratings in the required

data import format.
