I. ABANDONMENT OF ALL EASEMENTS LOCATED ON BIG CREEK DRIVE LOT 45, BLOCK "A"

ALSO KNOWN AS 7891 BIG CREEK DRIVE LOCATED IN SECTION TWO,BIG CREEK

SUBDIVISION:



Approval is requested to execute a Quit Claim Deed to James and Peggy Salie for

abandonment of a ten foot drainage and utility easement located on the rear

portion and the entire Storm Sewer Easement running along the eastern property

line of Lot 45, Block "A" also known as 7891 Big Creek Drive on Big Creek Drive

in Section Two Big Creek Subdivision to construct a storage building.





II. STREET ACCEPTANCE - THAT PORTION OF DOUBLE PINE DRIVE LOCATED IN FIRST

ADDITION TO SECTION ELEVEN - GARRETT PINES:



Approval is requested for the acceptance of a deed to that portion of Double

Pine Drive located in First Addition to Section Eleven, Garrett Pines. The

Engineering Department has inspected said streets and recommends the

acceptance. No city funds are involved until maintenance is assumed after the

two year warranty.





III. FEMA DISASTER ASSISTANCE:



Approval is requested to submit an application and if approved, accept

Public Assistance funds from the Federal Emergency Management Agency (FEMA) as

administered by the Georgia Emergency Management Agency (GEMA) for

reimbursement of disaster response and recovery expenses associated with the

Presidentially declared disaster caused by heavy rains and resulting erosion

and flooding from December 22, 2015 through January 13, 2016, and to amend the

budget by the amount reimbursed. The public assistance funding will range from

75% of eligible expenditures to 100% of eligible expenditures, depending on the

type of expenditure and the time reported. There is no cost to the city other

than those incurred costs that will be reimbursed.



IV. 2016 HOMELAND SECURITY GRANT PROGRAM - HAZMAT:



Approval is requested to accept a grant of $154,545, or as otherwise awarded,

from the 2016 Homeland Security Grant Program for the maintenance of Hazardous

Materials Equipment and to amend the Multi-Governmental Fund by like amount

with no matching funds required.





V. 2016 HOMELAND SECURITY GRANT PROGRAM - GEORGIA SEARCH AND RESCUE (GSAR):



Approval is requested to accept a grant of $117,320 or as otherwise

awarded, from the 2016 Homeland Security Grant Program the maintenance of

equipment received as part of the West Central Georgia Search and Rescue (GSAR)

Team and to amend Multi-Governmental Fund by like amount with no matching

funds required.





VI. JUVENILE JUSTICE INCENTIVE GRANT:



Approval is requested to submit a grant application and if awarded,

accept funds up to $750,000 from the Criminal Justice Coordinating Council for

the operation of the Muscogee County Juvenile Court and amend the

Multi-Governmental Fund by the amount of the grant award, with no local match

required. The grant period is from July 1, 2016 to July 30, 2017 and will be

used to provide evidence based treatment and surveillance to juvenile offenders.





VII. COCA-COLA BOTTLING COMPANY SERVE YOUR CITY GRANT:



Approval is requested to submit an application and if awarded accept

funds for $2,500 from Coca-Cola Bottling Company through a Serve Your City

Grant for our ?Little Free Library Program", with no matching funds required.

The books will be donated by the organizations in the community and will be

stored at one of our recreation centers.





VIII. GEORGIA RECREATION AND PARK ASSOCIATION INNOVATIVE GRANT:



Approval is requested to submit an application and if awarded accept

funds for $1,000 from Georgia Recreation and Park Association for our ?Little

Free Library Program, with no matching funds required. The books will be

donated by the organizations in the community and will be stored at one of our

recreation centers.



XI. 2015-2016 KNIGHT CITIES CHALLENGE GRANT:



Approval is requested to submit an application and if approved, accept

funds up to $229,950 or otherwise awarded from the Community Foundation of the

Chattahoochee Valley for The Columbus, GA, -based version of the 2015-2016

Knight Cities Challenge for Public Wi-Fi around and inside the Natatorium,

Plaza area of Government Center, and the area surrounding the Civic Center and

to amend the Multi-Governmental Fund by the amount of the grant awarded, with

no local match required.

1. ?CM/GC at Risk? Services for City Mill Riverwalk

A Mandatory Pre-Proposal Conference/Site Visit is scheduled for 2:00 PM

(Eastern) on Wednesday, April 13, 2016. Vendors shall convene in the office of

the project architect, Hecht Burdeshaw Architects, Inc., which is located at 8

West 11th Street, Suite 300, Columbus, GA 31901. The Site Visit will commence

immediately after the Pre-Proposal Conference. Vendors will be allowed a

10-minute grace period for the Pre-Proposal Conference. Any vendor who is not

present in the aforementioned location, within ten (10) minutes after the time

slated for the beginning of the Mandatory Pre-Proposal Conference, shall not be

allowed to participate any further in the RFP process.



April 14, 2016

1. Building Ethernet Wiring (Annual Contract) ? RFB No. 16-0031

A mandatory site visit will be held on April 14, 2016 at 10:00 AM. Vendors

shall convene at Fire Station #2, which is located at 1047 33rd Street,

Columbus, GA 31904. Each vendor who performs the mandatory site visit must

sign an attendance sheet, complete with the company?s name, attendee?s name,

address, and telephone & fax numbers. Any questions and/or concerns resulting

from the Site Visit must be submitted in writing to the Purchasing Division, as

specified on pages 8 and 9.

1. ?CM/GC at Risk? Services for City Mill Riverwalk

Scope of RFP

The Owner, by this Request for Proposals, desires to select a qualified

Contractor with the intent of both parties executing an Agreement for

Construction Services implementing the ?Construction Management/General

Contractor at Risk? project delivery method. The Owner has allocated a project

budget range of $1,100,000 to $1,500,000.



2. Advisory and Consulting Services for the Columbus Consolidated Government

457 Deferred Compensation Plan and Its Board (Annual Contract) ? RFP No. 16-0016

Scope of RFP

The Columbus Consolidated Government (CCG) seeks qualified firms to submit

proposals for providing professional advisory and investment consulting

services for the Columbus Consolidated Government?s 457 Deferred Compensation

Plan. The term of this contract will be for two years, with an option to renew

for three additional twelve-month periods.



3. Building Ethernet Wiring (Annual Contract) ? RFB No. 16-0031

Scope of Bid

Provide all tools and labor required to perform electrical services for

building Ethernet wiring at Fire Station #2 (1047 33rd Street), with potential

of many other locations in the next year. The contract period will be for two

(2) years with the option to renew for three (3) additional twelve-month

periods. Quotes will be obtained from the successful vendor, as needed, for

other locations.
