Authorization is requested to submit an application and if approved

accept a $98,480 grant from the Governor?s Office for Children and Families

(2010 System of Care: Community Strategy Grant Program) to fund a parent

II. ACQUISITION OF RIGHT OF WAY - MUSCOGEE TECHNOLOGY PARK PROJECT (SCHATULGA

RD./EASTERN CONN. FROM BUENA VISTA RD. TO SOUTH OF CHATTSWORTH RD.):



Authorization is requested to execute a Contract for Acquisition of Right of

Way Project for Muscogee Technology Park Project (Eastern Connector) with the

Georgia Department of Transportation (D.O.T.) for the construction and widening

of a New Road (Schatulga Rd./Eastern Conn. from Buena Vista Rd. to south of

Chattsworth Rd.) An agenda report, contract and resolution are attached.

III. STREET ACCEPTANCE - PORTION OF IVY RIDGE DRIVE, PORTION OF IVY PATCH DRIVE

AND PORTION OF BOSTON IVY LANE LOCATED IN SECTION EIGHT PHASE TWO, IVY PARK:



Authorization is requested to accept that portion of Ivy Ridge Drive,

Ivy Patch Drive and Boston Ivy Lane located in Section Eight Phase Two, Ivy

Park. The Department of Engineering has inspected sad streets and recommends

the acceptance. An agenda report, map and resolution are attached.

IV. EASEMENT FOR COLUMBUS WATER WORKS:



Authorization is requested to grant an easement to Columbus Water Works

on property located over and through the right of way of Part of Land Lot 59

1. Retail Space for Park and Ride at the Warm Springs Road/Manchester

Expressway Locations (Annual Contract) ? RFP No. 10-0011

Scope of RFP

The Columbus Consolidated Government seeks a retail/service business to occupy

space within the new (Rails-to-Trails) Park and Ride locations. The building,

which will have the same footprint and layout, are currently being constructed

at Manchester Expressway and Warm Springs Road (See attached site plan).

Successful respondents will provide a service or combination of services that

meet the needs of the City and surrounding market area. While the City would

prefer that the retail services be oriented around trail uses. Suggested

retail services could include but are not limited to; bike rental and retail,

convenience store (no gas sales), newsstand or other similar service.



March 3, 2010

1. Full Size 4x4 Pickup Trucks ? RFB No. 10-0042

Scope of RFB

Provide three (3) Full Size 4x4 Pickup Trucks to be used by the Columbus Office

of Homeland Security.

1. Employee Physicals (Annual Contract) ? RFP No. 10-0005

Scope of RFP

Provide a cost effective service plan for providing pre-employment physicals,

periodic employee physicals, drug/alcohol testing and related employee health

services on an ?as needed basis?. The contract term shall be for two years,

with the option to renew for three additional twelve-month periods. Contract

renewal will be contingent upon the mutual agreement of the City and the

Contractor.

1. Warm Springs Bike Trail: Cooper Creek Bridge & Flat Rock Park Bridge ? RFB

No. 10-0041

Scope of RFB

Provide construction of bridges at Cooper Creek and Flat Rock Park. The work

includes structural and site work in accordance with the designated plans and

specifications.

2. Lakebottom Park Track & Field Resurfacing and Trench Drain Installation ?

RFB No. 10-0040

Scope of RFB

The track and field resurfacing consists of providing a polyurethane running

track surface with structural spray finish, long jump/triple jump runway and

high jump area resurfacing and related work. The City will be responsible for

demolishing and removing the existing track.



The trench drain consists of installing a pre-cast, interlocking polyester

concrete trench drainage system around the entire track. The resurfacing

contractor shall be a member of the American Sports Builders Association.





March 12, 2010

1. Demolition Services (Annual Contract) ? RFP No. 10-0012

Scope of RFP

Provide all necessary equipment, materials, transportation, and labor for

residential and commercial demolition services on an ?as needed basis? for

Columbus Consolidated Government. The contract will be for two years, with the

option to renew for three (3) additional twelve-month periods. Contract

renewal will be contingent upon the mutual agreement of the city and the

Contractor.



2. Gym System (Re-Bid) ? RFB No. 10-0044

Scope of RFB

Provide and install a commercial grade gym system to Columbus Police

Department. The system shall consist of commercial grade strength equipment,

free weight equipment, treadmills, elliptical equipment, recumbent bikes and

magnetic cycles. For the strength equipment, vendors are requested to submit

bids for both belt systems and cable systems. The separate pricing is for

budgetary comparative purposes.



For uniformity and aesthetic purposes, the Columbus Police Department will only

consider commercial grade gym systems; bid submittals consisting of multiple

equipment manufacturers will not be considered. The exception will be the

dumbbells, plates and rack.

1. Lakebottom Park Walking Trail Improvements ? RFB No. 10-0043

Scope of Bid

Provide installation of park benches, trash receptacles, and concrete handicap

ramps. Additional work includes dumping, spreading and compacting M-10

screening material and replacing treated lumber used for trail edging.



All work shall be performed in accordance with the State of Georgia, Department

of Transportation ? Standard Specifications, Construction of Transportation

Systems, 2001 Edition or as Amended and in accordance with the contract

documents.
