(A) CHEVROLET TAHOE 2WD POLICE PURSUIT VEHICLES ? GEORGIA STATEWIDE CONTRACT



It is requested that Council approve the purchase of three (3) Chevrolet Tahoe

2WD Police Pursuit Vehicles from Hardy Chevrolet (Dallas, GA), in the amount of

$34,610.00 per unit, for a total price of $103,830.00, via Georgia State

Contract #99999-SPD-ES40199409-0002.



These vehicles will be used by the Columbus Police Department for Police

Services throughout the City. These vehicles will be purchased utilizing FY15

Salary Savings. They are replacement vehicles.



Bid specifications (RFB No. 16-0021) were posted on the web page of the

Purchasing Division, and the Georgia Procurement Registry. One (1) bid was

received on December 16, 2015. A phone survey of previous bidders determined

that no other vendors were interested in bidding on this item at the time.

This bid has been advertised, opened and reviewed. The bidder was:

Management Division recommends purchasing the vehicles via the Georgia State

Contract, and if approved by Council cancel Bid No RFB 16-0021.



Funds are budgeted in the FY16 Budget as follows: General Fund ? Police ?

(B) 2016 FORD F150 ? GEORGIA STATEWIDE CONTRACT



It is requested that Council approve the purchase of one (1) 2016 Ford F150

from Allan Vigil Ford (Morrow, GA), in the amount of $29,006.00, via Georgia

State Contract #99999-SPD-ES40199373-002.



This vehicle will be used by the Columbus Police Department for Police Services

throughout the City. This item will be purchased utilizing FY15 Salary

Savings. It is a new vehicle.



Funds are budgeted in the FY16 Budget as follows: General Fund ? Police ?

Motor Transport ? Light Trucks; 0101-400-3230-MOTR-7722.

(C) WASHER/EXTRACTOR FOR MUSCOGEE COUNTY JAIL



It is requested that Council approve the purchase of one (1) GA Braun

Washer/Extractor from Southeastern Laundry Equipment (Marietta, GA), in the

amount of $62,910.00.



The new Washer/Extractor, approved in the FY16 Budget, will replace a current

unit purchased in 2001. The old Washer/Extractor is beyond it?s useful life

due to the difficulty to obtain replacement parts when repairs are needed.



All Washer/Extractors utilized at the Jail and MCP are GA Braun equipment.

Facility Maintenance personnel are familiar with the equipment and have the

experience, skill and knowledge to make reasonable mechanical and electronic

repairs to the equipment with minimal impact to the budget. Southeastern

Laundry Equipment is the authorized seller of GA Braun equipment for the State

of Georgia.

(D) GEOTECHNICAL AND ENVIRONMENTAL TESTING SERVICES (ANNUAL CONTRACT) ? RFP NO.

16-0003



It is requested that Council authorize the execution of an annual contract with

ATC Group Services, LLC (Columbus, GA), Building and Earth Sciences, Inc.

(Columbus, GA), MC Squared, Inc. (Kennesaw, GA) and Terracon Consultants, Inc.

(Kennesaw, GA) for geotechnical environmental testing services on an as-needed

basis.



The geotechnical and environmental testing services are required by the

Engineering Department for soil and construction material testing during the

development or design of various construction projects. This contract may also

be utilized by any other City agency requiring the services.



The initial term of the contrast will be for two (2) years with the option to

renew for three (3) additional twelve-month periods. Contract renewal will be

contingent upon the mutual agreement of the City and the Contractor.



Annual Contract History:

The previous five-year contract was awarded on November 23, 2010 (Resolution

No. 425-10) to ATC Associates, Inc. (Columbus, GA) and ECS Southeast, LLC

(Marietta, GA).



RFP Advertisement and Receipt of Proposals:

RFP specifications were posted on the web pages of the Purchasing Division and

the Georgia Procurement Registry on August 1, 2015.



This RFP has been advertised, opened and evaluated. Proposals were received on

September 4, 2015 from the following five (5) vendors:

MC Squared (Kennesaw, GA)

Terracon Consultants, Inc. (Kennesaw, GA)

The following events took place after receipt of proposals:

the discussions, the voting members of the committee confirmed they were ready

to perform their individual evaluations

recommended award to the the four highest rated firms.





Evaluation Committee:

Proposals were reviewed by members of the Evaluation Committee, which consisted

of one voting member from the Engineering Department, one voting member from

the Inspections and Code Department, and one voting member from the Public

Works Department. An additional representative from the Engineering

Department served as a non-voting advisory member.



Award Recommendation:

The evaluation committee deemed the proposals from ATC Group Services, LLC,

Building and Earth Sciences, Inc., MC Squared, Inc. and Terracon Consultants,

Inc. as most responsive to the RFP specifications. Therefore the Evaluation

Committee, as reflected by their comments provided below, recommends award of

the contract to the firms for the following reasons:



ATC Group Services

? The firm has satisfactorily provided geotechnical and environmental testing

services in the past, and looks out for the City?s best interest by often

bringing alternate suggestions to the table.

? The firm worked with the City by completing the underground storage tank

issue at the Cusseta Road fuel pump that was initially awarded to another firm.

? The firm has their own lab for testing materials.

? The firm has experience with both public and private firms, for services

ranging from geotechnical to underground storage and hazardous materials. They

are qualified for air monitoring.

? The firm meets the pre-qualifications of Georgia Department of Transportation

(GDOT)



Vendor Experience/Qualifications

? ATC Group Services has been in Georgia since 1973 and opened its Columbus

office in 2009.

? Local projects include:

o Columbus Consolidated Government - City Service Center Complex

? Construction materials testing and special inspections in accordance with

ASTM and ICC building codes including: foundation installation monitoring,

geopier evaluations, soils/earthwork, concrete, reinforcing steel, masonry,

structural steel, asphalt testing, and floor flatness/levelness evaluations.

o Columbus Consolidated Government - Cusseta Road Fuel Depot

? Health and safety plan, remediation system installation, drilling, well

installation, soil and groundwater sampling, certified laboratory analyses,

CAP-Part B addendum report, quarterly monitoring reports, GA EPD regulatory

correspondence. Submitted the Corrective Action Plan in August 2011, installed

the remediation system in June 2012, and maintained the remediation system and

provided compliance monitoring from June 2012 ? October 2013. By October

2013, the vendor was performing post-remediation groundwater monitoring. In

December 2014, a ?No Further Action Status? was granted for release.

o Columbus State University - Student Recreation Center

? Construction materials testing and special inspections including: foundation

installation monitoring, soils/earthwork, concrete, reinforcing steel, masonry,

structural steel, fireproofing evaluation, asphalt testing, and roofing

? The firm?s Columbus laboratory facility is accredited by AASHTO and meets the

requirements of ASTM E329

? ATC Group Services is currently approved with GDOT for area class codes

6.01a, 601b, 6.02 and 6.05. Additionally, the firm has met the requirements

for area class 6.04a Laboratory Testing and anticipates approval soon.

? The Operations Manager for the Columbus office has held the GDOT Roadway

Testing Technician (RTT) Certification and is currently in the renewal process.

? All field technicians that perform construction materials testing are issued

a company truck, outfitted with safety gear, first aid kits, fire

extinguishers, and a standard inventory of equipment used for soils, concrete

and asphalt testing. They are also issued a laptop to produce their Field

Reports.

? The geotechnical engineers are outfitted with portable cone pentameter

equipment for evaluating soil subgrade conditions in the field. In addition,

geotechnical equipment in-house includes:

- Flat Plate Dilatometer (settlement, other evaluations)

- ReMi Micro Tremor Refraction Survey Equipment (shear wave velocity testing)

? ATC Group Services keeps licensing current for specific geotechnical software

for various calculations, including shallow foundation settlements, deep

foundation capacities, lateral loading of plies/shafts, and various pavement

assessments, as well as, for seismic wave velocity testing and producing

geotechnical reports.

they need for a hazardous material site survey ? asbestos, microbial or

field to the Project Manager for review, and upon final review/approval, email

clouding, giving the Project Team 27/7 access. Open items automatically filter

on a daily bases to ensure timely communications. RFIs, project specifications

and plans can be uploaded by the Project Team for a consistent database.

? Terracon has extensive experience performing underground storage tank

corrective action projects under the supervision of the GA EPD Underground

? Terracon employs an innovative system that includes Microsoft Office Outlook

and their proprietary Construction Materials Laboratory Management System

(CMELMS) software that will be used for scheduling and tracking work tasks for

field personnel. The program automatically tracks all reports and provides

immediate availability of test results, a copy of the report can be sent

electronically via email to the City within 48 hours.

? Terracon uses mobile communication technology on a laptop computer to prepare

standardized reports, create sketches, annotate project drawings and prepare

photos with captions. This process is used for project reporting on al SI and

CMT projects.



The City?s Procurement Ordinance Article 3-110, Competitive Sealed Proposals

for the procurement of Equipment, Supplies and Professional Services, governs

the RFP Process. During the RFP process, there is no formal opening due to the

possibility of negotiated components of the proposal. In the event City

Council does not approve the recommended offeror, no proposal responses or any

negotiated components are made public until after the award of the contract.

Therefore, the evaluation results and cost information has been submitted to

the City Manager in a separate memo for informational purposes.



Funding will be allocated in the budget for specific projects through the CIP,

SPLOST, Bond, etc.

(E) TREATMENT PROVIDERS FOR MENTAL COURT AND VETERANS TREATMENT COURT (ANNUAL

CONTRACT) ? RFP NO. 15-0032



It is requested that Council authorize the annual contract with Pastoral

Institute, Inc. (Columbus, GA) to provide treatment for Individual Post

Traumatic Stress Disorder (PTSD) Counseling; and authorize the annual contract

Shelley Willetts Reed (Columbus, GA) to provide treatment for Psychological

Assessment to Mental Health Court and Veterans Treatment Court participates.



The initial term of this contract shall be for two (2) years with the option to

renew for three (3) additional twelve-month periods.



Annual Contract History:

This will be the first time Mental Health Court and Veterans Treatment Court

has requested an annual contract for these services.

Letter sent to the two recommended vendors informing of the pay rate change as

prescribed by the Criminal Justice Coordinating of Council of Georgia. Both

vendors concurred with the contract change.



Evaluation Committee:

Proposals were reviewed by the Evaluation Committee members, which consisted of

one voting member from the Muscogee County Mental Health and Veterans Treatment

Court, one voting member from AmericanWork, Inc., and one voting member from

the Advisory Board Member for Muscogee County Mental Health and Veterans Court.



Award Recommendation:



Individual Post Traumatic Stress Disorder (PTSD) Counseling

The evaluation committee deemed the proposal from Pastoral Institute, Inc.,

most responsive to the RFP specifications for Individual Post Traumatic Stress

Disorder (PTSD) Counselor. Therefore, the Evaluation Committee, as reflected

by their comments provided below, recommends award of the contract to Pastoral

Institute for Individual Post Traumatic Stress Disorder (PTSD) Counseling for

the following reasons:



? The Pastoral Institute is highly qualified in PTSD counseling to health care

patients.

? Excellent qualifications for counseling; extensive training and expertise in

PTSD.

? Present and past history of serving over 1000 military personnel. Star

certification working with military families.

? They have an excellent service plan, which provides treatment for military

personnel and their families. They offer an array of treatment modalities to

include one of the few agencies providing PTSD specific counseling.

? They have ten plus years working with courts. They are presently working

with vets, excellent understanding of military culture, able to work with

family issues, and knowledge of Mental Health Substance Abuse treatment as well.



Pastoral Institute, Inc.



Vendor Experience/Qualifications:



? The Pastoral Institute has experience in managing federal, state, and local

government contracts and grants. Ample financial controls are in place based on

generally accepted accounting principles. There are six divisions within the

organization.

? The Pastoral Institute is uniquely qualified to provide treatment to

individuals going through Mental Health Court and Veterans Court.

? In 2014, the Pastoral institute provided counseling services to over 1,000

military service members or their families.

? The clinicians on staff have received specialized training equipping them to

provide the best evidence-based therapy techniques for service members. Last

year the Pastoral Institute hosted Coming Home: a seminar dedicated to the

challenges faced by service members and their families. The seminar was for

clergy members and clinicians and focused on how communities can better

understand the challenges that veterans experience due to living with a high

operations tempo and multiple combat deployments. Topics included recognizing

and treating

moral injury and Post Traumatic Stress Disorder.

? Also in 2014, the Pastoral Institute partnered with STAR Behavioral Health to

host Tier I and II trainings. Tier III training is scheduled for July 2015.

STAR Behavioral Health is a resource for veterans, service members, and their

families to locate mental health providers that have specialized training in

understanding and treating military service members and their families.

Clinicians learned basic knowledge of military culture and covered diagnoses

such as Traumatic Brain Injury and substance abuse as well as other mental

health diagnoses that service members face such as sleep disorders.

? The Pastoral Institute has also worked with the U.S. Department of Probation,

U.S. District Court, Middle District Court for a decade to evaluate and treat

mental health, drug abuse, or alcohol abuse disorders.



Client Work History

? Chaplain Family Life Training Program ? Services Provided: The Pastoral

Institute, by contract, has provided supervision and training of counseling

interns, as well as peer supervision for Directors.

? US Department of Probation ? Services Provided: Working in conjunction with

U.S. Federal Probation Officers, the Pastoral Institute provides court ordered

behavioral health treatment for paroles (and their families) who have been

incarcerated in the U.S. Federal court system.

? Open Door Community House ? Services Provided: The Pastoral Institute provides

contracted counseling for the residents in homeless shelters affiliated with

the Community Case Management Program administrated by Open Door Community

House.



Psychological Assessment

The evaluation committee deemed the proposal from Shelley Willetts Reed most

responsive to the RFP specifications for Psychological Assessment. Therefore,

the Evaluation Committee, as reflected by their comments provided below,

recommends award of the contract for Psychological Assessment to Shelley

Willetts Reed for the following reasons:



? Excellent experience conducting Mental Health assessments for Mental Health

courts.

? Performing assessments and working with Mental Health Veteran court.

? They have a good history of working with the Mental Health Court Program.

She is local and is able to provide the required testing. She is a professor

of Assessments and Evaluations at Troy University.

? They have fourteen years experience working in Columbus with Mental Health

Veteran courts, conducting assessments and they have a licensed PhD, and a

Licensed Professional Counselor/Certified Addiction Counselor.

Vendor Experience/Qualifications:



? Provided Mental Health and Substance Abuse Counseling and Assessment for over

14 years.

? Currently employed as an Assistant Professor with Troy University in the

Graduate Counseling Program.

? Has a PhD in Counseling Education and Supervision.

? Has taught numerous evaluation and assessment courses. Biopsychosocial

assessment, formal standardized psychological testing instruments, and thorough

interviews are all part of accurately diagnosing and treating persons with

mental health disorders. Conducting a thorough assessment is critical to

providing proper treatment recommendations to ensure the client receives the

best individualized treatment plan they can for the best prognosis for

treatment progress and compliance within the court programs.

? Has a long history of conducting such assessments both within Mental Health

Court, Inpatient, and Outpatient settings for those diagnosed with mental

health disorders.

? By conducting a formal assessment she can assist the client in getting the

most effective and appropriate treatment to include but not limited to:

individual counseling, family counseling, court supervision, linkage to

community resources, and case management. By addressing the unique needs of

the client within the criminal justice system the individualized assessment and

treatment plan can guide this process to benefit not only the Mental Health and

Criminal Justice systems but ultimately the client.

(F) AMEND CONTRACT TERM FOR ELEVATOR MAINTENANCE SERVICES (ANNUAL CONTRACT) -

RFB NO. 12-0038



It is requested that Council amend the annual contract, for elevator

maintenance service, with Georgia Elevator Company d/b/a Elevator Service

Company (Columbus, GA) by adding two additional years to the contract term.



Per Resolution #183-15 (approved June 23, 2015) Council authorized the

execution of an annual contract with Georgia Elevator Company d/b/a Elevator

Service Company (Columbus, GA) to assume the remaining term of the Elevator

Maintenance Service annual contract. The contract was originally awarded to

Mowrey Elevator Company, who had failed to fulfill the requirements of the

contract. At that time the remaining term of the contract was one year with

the option to renew for two additional twelve-month periods. However, due to

some major maintenance that is required, the Public Works Department is

requesting that the contract term be amended to add an additional two years to

the contract, thereby giving the vendor a full five-year contract.



Therefore, the contract term of one year with the option to renew for two

additional twelve month periods would be changed to read as one year with the

option to renew for four additional twelve-month periods. Contract renewal

will be contingent upon the mutual agreement of the City and the Contractor.

(G) CHANGE ORDER 1 FOR SOUTH LUMPKIN MULTI-USE TRAIL ? RFB NO. 15-0025



It is requested that Council authorize Change Order 1 with Alexander

Contracting Company, Inc. (Fortson, GA) in the amount of $272,504.78 for the

South Lumpkin Multi-Use Trail project.



Per Resolution No. 120-15, Council authorized the construction of the multi-use

trail, beginning on Matheson Road and ending at Cusseta Road. The original bid

was for the construction of the multi-use trail and two (2) trail head

buildings; however, all bids exceeded the amount budgeted for the project.

Therefore, the City requested Alexander Contracting Company, the low bidder, to

value engineer the project by removing the line items for the two (2) trail

head buildings.



Alexander Contracting Company?s original bid included the cost for a

subcontractor to construct the (2) buildings, provide signs and provide

pillars. Because the (2) buildings were deleted during the value-engineering

process, Alexander Contracting Company documents the subcontractor was not

interested in constructing the balance of the scope they priced. Therefore,

the Engineering Department requested a quote from Alexander Contracting Company

for the signage.



The primary need for Change Order 1 is for cost adjustments of the signage on

the site, bad soils that required undercutting, and resizing of pipes to

prevent runoff onto private property.
