I. U. S. CONFERENCE OF MAYORS (USCM) AND AMERICAN BEVERAGE ASSOCIATION (ABA) --
CHILDHOOD OBESITY PREVENTION AWARDS PROGRAM:
Authorization is requested to submit an application and, if approved,
accept a $120,000, or as otherwise awarded, grant with no local match from the
U.S. Conference of Mayors and their partner the American Beverage Association.
This grant will be used to provide funding to prevent childhood obesity through
programs of Live Healthy Columbus. An agenda report and resolution are

II. SPRING FLING CARNIVAL EXTENDED CONTRACT:
Authorization is requested to enter into a five year agreement with
Drew Expositions, Event Coordinators Inc., of Augusta, GA, for Coordinating
Services for the annual Spring Fling Carnival. An agenda report, contract, and

III. HEALTH INSURANCE - ACTIVE/PRE-65 RETIREES:
Authorization is requested of rate changes for Health Insurance for
Active Employees, Pre-65 Retirees and modifications to the plan benefits. An

Scope of Bid
Provide hot asphalt and general road repair services to include: removing old
asphalt, resurfacing roads, asphalt patching, concrete patching, mill asphalt
paving and etc., on an ?as needed? basis, to the Columbus Consolidated
Government (the City). The contract term will be for three years, with the
option to renew for two additional twelve-month periods.
