I. DIRECTOR OF INFORMATION TECHNOLOGY:



Authorization is requested for the appointment of Dr. Forrest Toelle as

Director of Information Technology Department. A resolution and resume are

II. INTERGOVERNMENTAL AGREEMENT WITH DEVELOPMENT AUTHORITY - AMENDMENT:



Authorization is requested to execute an amendment to the Intergovernmental

Agreement (Resolution #234-09) with Development Authority of Columbus, Georgia

to extend remaining terms of the agreement from 5 years to 10 years and reduce

III. PROJECT LAUNCH HOME VISITATION GRANT - COOPERATIVE EXTENSION SERVICES:



Authorization is requested to submit an application and if approved accept a

grant (with no local match required) from the Columbus Board of Health for up

to $46,000 from October 1, 2014 through September 30, 2015 and amend the

Multi-Governmental Fund by the award of the grant. An agenda report and

IV. FY15 LOCAL GOVERNMENT & IMPROVEMENT GRANT (LMIG) - RIVER ROAD/BRADLEY PARK

DRIVE ROUNDABOUT:



Authorization is requested to accept a Local Government & Improvement Grant

(LMIG) for $1.5 million from the Georgia Department of Transportation (GDOT)

for the River Road/Bradley Park Drive Improvement Project. An agenda report,

V. STREET ACCEPTANCE - CAL COURT LOCATED IN SEARS POND SOUTH:



Authorization is requested for the acceptance of a deed to Cal Court

located in Sears Pond South. The Engineering Department has inspected said

street and recommends the acceptance. An agenda report, map and resolution are

VI. QUIT CLAIM DEED ACCEPTANCE /DESIGNATION -- ALVAH H. CHAPMAN WAY AND PORTION

OF ALBION WAY:



Authorization is requested for the acceptance of a Quit Claim Deed to

Alvah H. Chapman Way, portion of Albion Way and designation of Alvah H. Chapman

Way/portion of Albion Way as public streets. An agenda report and resolution

VII. CHARGING STATION GRANT - 11TH STREET AND BROADWAY:



Authorization is requested to submit an application and, if awarded, accept a

$50,000 grant with a required $5,000, which is 10% of the local match from the

Georgia Environmental Financial Association for installation costs of a vehicle

charging station to be donated by Nissan and to amend the multi-governmental

fund by the amount of the award and local match. An agenda report and

1. Riverwalk Project-14th Street Plaza ? RFB No. 15-0006

Scope of Bid

Provide construction of the proposed Riverwalk Plaza located on 14th Street

between Broadway and the 14th Street Bridge in Columbus, Georgia. The work

primarily involves demolition of existing street pavement and concrete curb and

gutters and construction of a vehicular and pedestrian friendly Plaza area at

grade with landscaping and site furnishings.



The governing specifications for this project, including such items as bidding

requirements, general conditions, technical specifications and related items,

shall be in accordance with the State of Georgia, Department of

Transportation-Standard Specifications, Construction of Transportation Systems

and Supplemental Specifications, 2001 Edition or as amended.



October 17, 2014

1. Secure Shredding Services (Annual Contract) - RFP No. 15-0013

Scope of RFP

The Consolidated Government of Columbus, Georgia (the City) is requesting

proposals from qualified vendors to provide secure shredding services. The

contract period will be for two (2) year with the option to renew for three (3)

additional twelve-month periods.

1. Valencia Drive Culvert Replacement - RFB No. 15-0009

Scope of RFP

The work shall consist of removing and replacing an existing double barrel

concrete box culvert located on Valencia Drive and replacing it with a triple

barrel concrete box culvert. The work shall also include placement of concrete

curb and gutter, curt inlet, asphalt paving, pavement markings, installation of

guardrails, and concrete sidewalks.



November 26, 2014

1. Redevelopment Plan for Claflin School ? RFP No. 14-0002

Scope of RFP

The City of Columbus is requesting proposals from experienced, creative

architects, building and real estate development firms OR a philanthropic

individual or nonprofit group to present a redevelopment plan for adaptive

reuse of the Claflin School building that will be consistent with applicable

deed restrictions and preserve the historic site.
