I. CONDEMNATION ON PARCELS 19 AND 20 - WYNNTON ROAD MASTER PLAN - PHASE 1:



Approval is requested for the acquisition of parcel 19 and 20, 2435

Wynnton Road for the Wynnton Road Master Plan - Phase I, from Cedar Avenue to

Hilton Avenue/13th Street for a temporary easement for county road and/or

municipal street purposes and authorize the filing of a Declaration of Taking.

This project is funded 80% by the Georgia Department of Transportation and 20%

by Midtown Incorporation. The city will be responsible for service and upkeep

after completion of the project.





II. FOLLOW ME TRAIL:



Approval is requested to name the South Lumpkin Rails to Trails

Transportation Investment Act (TIA) project the "Follow Me Trail" and to allow

the ordering of the necessary signage for the trail.





III. ACCOUNTABILITY COURT CASE MANAGEMENT SYSTEM:



Approval is requested to purchase the Accountability Court Case

Management System from FivePoints Solutions, LLC. and allow Presiding Judge

Warner L. Kennon to sign the statement of work, with no additional expenses and

no match requirements.





IV. LEARN TO SWIM DONATIONS:



Approval is requested to allow Columbus Parks and Recreation to accept

donations in the form of gift certificates, checks, and cash from sponsors and

individuals that will be spent on any items or materials during the duration of

the Learn to Swim Program which is used to help teach kindergarten students the

necessary water safety and swimming skills at the Columbus Aquatic Center.

1. 5-Year Consolidated Plan, 1st Year Annual Action Plan and Analysis of

Impediments to Fair Housing ? RFP No. 16-0002

Scope of RFP

Columbus Consolidated Government is requesting proposals for consulting

services for the preparation of its 5-year Consolidated Plan, 1st Year Annual

Action Plan and its Analysis of Impediments to Fair Housing (AI) documents. The

selected firm will develop and carry out the process for completing a 5-year

Consolidated Plan for a successful submission to HUD.



2. Emergency Clean-Up and Debris Removal (Annual Contract) ? RFP No. 16-0005

Scope of RFP

Columbus Consolidated Government is requesting proposals qualified firms to

submit proposals to provide services for emergency clean-up and removal of

debris that may result from natural (storms, fallen trees, etc.) or man-made

disasters (terrorism, remnants of destroyed buildings, etc.) on an ?as needed?

basis. The term of this contract will be for two years, with an option to

renew for three additional twelve-month periods.



September 4, 2015

1. Security Surveillance Equipment ? Department of Transportation/METRA (Annual

Contract) ? RFP No. 16-0001

Scope of RFP

It is the intent of Columbus Consolidated Government (the City) to enter into a

contractual agreement with a qualified contractor for the provision and

maintenance services of Security Surveillance Equipment for the Department of

Transportation/METRA. Quotes will be obtained from the successful vendor(s),

as needed for other departments. The term of this contract shall be for one

year, with the option to renew for two (2) additional twelve-month periods.



2. Geotechnical & Environmental Testing Services (Annual Contract ) ? RFP No.

16-0003

Scope of RFP

Provide geotechnical and environmental testing services on an as-needed basis

to the Engineering Department for soil and construction material testing for

the development or design of various construction projects. This contract may

also be utilized by any other City agency requiring the services. The term of

the contract will be two (2) years with the option to renew for three (3)

additional twelve-month periods.
