SUBJECT: Aflac Foundation Grant for Wellness Program

Recommendation: Approval is requested to submit an application and, if
approved, accept a grant for $10,000 or as awarded from the Aflac Foundation to
enhance the Columbus Consolidated Government?s Wellness Program, and to amend
the Multi-governmental Fund by the amount awarded. The purpose of the wellness
program is to provide opportunities for employees to improve their overall
health and wellness, increase employee awareness of the value of staying
healthy, and to control rising healthcare costs incurred because of employee
injuries and illnesses.
Background: The CCG Wellness Program is an investment in the well being of its
employees and it focuses on preventive health. The CCG has incorporated
wellness initiatives in the workplace for several years now including an
employee assistance program, fitness walks, smoking cessation, Weight Watchers
at Work Program and etc.
Analysis: A grant award from the Aflac Foundation will allow the CCG to enhance
its current program with a consistent proactive strategy of employee
engagement. The CCG?s wellness initiatives fit into Aflac?s community focus
area of health.
Financial Considerations: The grant is for $10,000 or amount awarded with no
matching funds required.
Legal Considerations: The Columbus Consolidated Government is eligible to apply
for this grant award with Council approval.
Recommendation/Actions: Authorize the City Manager to submit and, if approved,
accept a grant for $10,000 from the Aflac Foundation and amend the
Multi-governmental Fund by the amount of the award.

A RESOLUTION AUTHORIZING THE CITY MANAGER TO SUBMIT AN APPLICATION AND, IF
APPROVED, ACCEPT A GRANT FROM THE AFLAC FOUNDATION FOR $10,000, OR AS OTHERWISE
AWARDED, WITH NO LOCAL MATCH REQUIRED, TO PROVIDE OPPORTUNITIES FOR EMPLOYEES
TO IMPROVE THEIR OVERALL HEALTH AND WELLNESS, INCREASE EMPLOYEE AWARENESS OF
THE VALUE OF STAYING HEALTHY, AND TO CONTROL RISING HEALTHCARE COSTS INCURRED
BECAUSE OF EMPLOYEE INJURIES AND ILLNESS, AND TO AMEND THE MULTI-GOVERNMENTAL
FUND BY THE AMOUNT OF THE AWARD.
WHEREAS, a grant award from the Aflac Foundation will benefit 3,000 public
employees and will provide them an opportunity to improve their overall health
and well being; and
WHEREAS, the Columbus Consolidated Government?s Human Resources Department will
implement a sound wellness program that focuses on actions that will translate
into employee behavior change and provide measurable results.
NOW, THEREFORE, THE COUNCIL OF COLUMBUS, GEORGIA HEREBY RESOLVES AS FOLLOWS:
That the City Manager is hereby authorized to submit an application and, if
approved, accept a grant from the Aflac Foundation for $10,000 or the amount
awarded to enhance the Columbus Consolidated Government?s Wellness Program to
improve the health and well being of CCG employees.
