II. LEASE AGREEMENT WITH COLUMBUS REGIONAL TENNIS ASSOCIATION (CORTA):



Authorization is requested to execute a 10 Year Lease Agreement with Columbus

Regional Tennis Association (CORTA) for an annual fee of $1.00 per year. An

III. CELL TOWER LEASE EXTENSION - SBA COMMUNICATIONS CORPORATION:



Authorization is requested to execute a ground lease extension with SBA

Communications Corporation on the Schatulga Road tower lease GA09926-A for two

IV. TIMBER CREEK ESTATES - JOINT USE ACCESS EASEMENT:



Authorization is requested of a variance to City Code Section 7.4.2. to

allow a Joint Use Access Easement to serve Lots 72, 73, 74 and 75, in Timber

Creek Estate subdivision, for driveway access to Heiferhorn Way. An agenda

1. Motorcycle Radar Packages ? PQ No. 12-0004

Scope of Bid

Provide seven (7) Bee III Motorcycle Ka-Band Dual Antenna Radar Packages, or

equivalent, for Harley Davidson ?Road King? motorcycles: six (6) units for 2011

model, one (1) unit for 2012 model. Radar packages are to include: radar,

waterproof antenna, mounting brackets, wired remote, two certified tuning

forks, cables, operators manual and a 2-year warranty. Motorcycle radar

packages are to be used by the Columbus Police Department.



2. Stump Grinder ? RFB No. 12-0021

Scope of Bid

Provide one (1) Deutz Diesel Stump Grinder, Model 7500 with 79 HP, or

equivalent, to be used by the Urban Forestry and Beautification Right-of-Way

Maintenance Division of the Public Services Department for grinding stumps from

City right-of-ways and other City properties.



3. ? Ton Crew Cab, 4x4 Pickup Trucks ? RFB No. 12-0023

Scope of Bid

Provide three (3) ? Ton Crew Cab, 4x4 Pickup Trucks, to be used by various

departments of the City.

1. Mental Health Services for Muscogee County Jail (Annual Contract) ? RFP No.

12-0006

Scope of RFP

Provide comprehensive Mental Health Services for inmates at the Muscogee County

Jail, that meet or exceed the requirements of the 8Th amendment of the United

States Constitution and are commensurate with the services accessible within

our community.



2. Real Estate Appraisal Services (Annual Contract) ? RFP No. 12-0011

Scope of RFP

Provide real estate appraisal services for various projects involving land

acquisition and disposition on an ?as needed basis?. The initial term of the

contract period shall go through May 24, 2012, with the option to renew for

three (2) additional twelve-month periods. Contract renewal will be contingent

upon the mutual agreement of the City and the Contractor.



January 4, 2012

1. Sand Spreaders ? RFB No. 12-0026

Scope of Bid

Provide two (2) Hi-Way 10' E2020XT Sand Spreaders, or equivalent, to be used by

the Right of Way Maintenance Division, of the Public Services Department, to

spread sand during inclement weather. Sand Spreaders must work with existing

trucks: 2009 Peterbuilt 14-Yard Dump Truck and 1997 Ford F&B Series 7-Yard Dump

Truck. Installation and/or technical assistance on-site may be required.



January 6, 2012

1. Debt Collection Services (Annual Contract) ? RFP No. 12-0009

Scope of RFP

Provide debt collection services on an ?as needed basis? for, but not limited

to, Landfill Collections, Parking, False Alarms, Ambulance, Demolition Liens,

etc. The contract term shall be for two years, with the options to renew for

three (3) additional twelve-month periods. Contract renewal will be contingent

upon the mutual agreement of the City and the Contractor.

1. On-Site Clinic for Employees, Retirees, & Dependants (Annual Contract) ? RFP

No. 12-0010

Scope of RFP

Contract with a clinic provider to establish an on-site or nearby clinic for

employees, retirees and dependants. The contract term shall be for two years,

with the option to renew for three additional twelve-month periods. Contract

renewal will be contingent upon the mutual agreement of the City and the

Contractor.
