I. FY2013 GRANT APPLICATION FOR CAPITAL AND PLANNING ASSISTANCE:



Authorization is requested to submit an application and if approved

accept $1,490,314 in funding from the Federal Transit Administration (FTA) and

the Georgia Department of Transportation (GDOT) for capital, planning and

operating grants. An agenda report and two resolutions are attached. (1)

Deputy City Manager Lisa Goodwin will provide a Stimulus Report.





EMERGENCY PURCHASE ? INFORMATION ONLY



CHEROKEE RETAINING WALL FAILURE:



In early January 2012, a retaining wall collapsed on Cherokee Avenue. On

January 10th, Council was made aware of the emergency and notified of the

intent to proceed with repairs under an emergency basis.



On January 13th, the Engineering Department received quotes from the following

vendors:

KAM, Inc. (Columbus, GA)$ 71,000.00

Southeastern Site Development (Newnan, GA) 118,295.00

Robinson Paving (Columbus, GA) 119,888.00

Cline Service Corporation (Columbus, GA) 216,925.00



The vendor will replace the collapsed section of the concrete channel wall

located along Weracoba Creek, and adjacent to Cherokee Avenue, with a new

gabion wall section. The low bidder expects to complete the work within

approximately fifteen days. The City Manager approved the emergency repair on

January 17, 2012.



Funds in the amount of $75,000.00 (to cover the quoted amount, plus

contingencies) are available in the FY12 Budget: Special Project/Capital

Developer/Operator for an RV Park and/or Marina at Oxbow Meadows Environmental

Park ? RFQ No. 12-0002

A Non-Mandatory Pre-Proposal Conference Visit will be held at 11:00 AM on

Wednesday, January 25, 2012 in the Oxbow Meadows Environmental Learning Center,

which is located at 3535 South Lumpkin Road, Columbus, GA. A tour of the site

will follow the meeting.



January 27, 2012

1. On-Site Clinic for Employees, Retirees, & Dependants (Annual Contract) ? RFP

No. 12-0010

Scope of RFP

Contract with a clinic provider to establish an on-site or nearby clinic for

employees, retirees and dependants. The contract term shall be for two years,

with the option to renew for three additional twelve-month periods. Contract

renewal will be contingent upon the mutual agreement of the City and the

Contractor.



2. Personal Property & Residential Mail/Processing for Tax Assessors Office

(Annual Contract) ? RFP No. 12-0012

Scope of RFP

Provide printing and mailing service to The Muscogee County Board of Tax

Assessors, the required service include following:

1. Chattahoochee Riverwalk Bank Stabilization ? RFB No. 12-0031

Scope of Bid

The work includes performing slope stabilization activities inclusive, but not

limited to: utility location; grading; installation of bioengineering (use of

geogrid, soil anchoring, and turf reinforcement matting); erosion and sediment

control; and detailed landscape treatments.



2. Pedestrian Bridge Painting Project ? RFB No. 12-0032

Scope of Bid

This work will consist of sandblasting and recoating each bridge with a high

performance epoxy coating, re-weld connections as required, remove and dispose

of existing fence material, install new vinyl coated chain link fence fabric,

reinforce members as required, and repair additional stem walls to both South

Lumpkin Road and Buena Vista Road sites.

Scope of Bid

Provide landscape material consisting of pine straw, wheat straw and hay on an

departments. The contract term will be for three years.

1. Audit Services of Contract for Hospital Care to Indigent Persons ? RFP No.

12-0014

Scope of Proposal

Columbus Consolidated Government is soliciting proposals for the professional

services of a Certified Public Accountant to perform a compliance audit of the

hospital care to indigent persons contract between the Columbus Consolidated

Government and the Medical Center Hospital Authority.

Audio/Video Court Recording System to be used by the Juvenile Court System.

1. Developer/Operator for an RV Park and/or Marina at Oxbow Meadows

Environmental Park ? RFP No. 12-0008

Columbus Consolidated Government is looking for qualified developers/operators

to submit a proposal for the development of certain City-owned property located

in Oxbow Meadows Environmental Park. The success of the City?s 16-mile

Riverwalk project has prompted interest by the City to target portions of

vacant Oxbow Meadows land for additional community improvements.
