I. DONATION OF A FITNESS PARK AT ROTARY PARK:



Approval is requested authorizing the acceptance of a donation of a

Fitness Park from Together 2017, valued at $73,650.00 placed at Rotary Park and

will be known as Fitness Together@Rotary Park.





II. DONATION FOR COOPER CREEK TENNIS CENTER:



Approval is requested to accept a donation of $25,000 given by an

anonymous donor for Cooper Creek Tennis Center to be used for items, supplies

or equipment needed at the Tennis Center.





III. DONATION FOR FIRE/EMS:



Approval is requested to accept the donation of 74 Self Contained

Breathing Apparatus (SCBA?s), 100+ Masks, and Miscellaneous Parts from the Fort

Benning Fire Department. This donation is part of the ?Public to Public

Initiative? through the Federal Government which allows government agencies to

donate items to local jurisdictions.





IV. CELL PHONE LEASE TOWER RENEWAL - 8889 RIVER ROAD:



Approval is requested to execute a five (5) year property lease renewal

with Verizon Wireless for the purpose of leasing space at 8889 River Road to

operate and maintain an existing telecommunications tower. The lease renewal

amount will be increased to $34,419.44 per year which is received in quarterly

payments of $8,604.86.





V. FAMILY DRUG COURT GRANT:



Approval is requested to apply for and to accept, if awarded, a

Substance Abuse and Mental Health Services Administration (SAMHSA) grant award

of $425,000 per year for five years, from July 1, 2018 to June 30, 2023, or as

otherwise awarded, with no local match requirement, for the purpose of funding

the Muscogee County Family Drug Court. The grant of $425,000 per year will

result in a total of $2,125,000 received for the five-year period. Approval is

also requested to amend the Multi-governmental Fund by the amount awarded.

1. Water Management Services (Annual Contract) ? RFB No. 18-0026

Scope of Bid

Provide a comprehensive water management program for the cooling

towers, boilers and closed loop systems operated by Columbus Consolidated

Government. The contract term will be for two (2) years, with the option to

renew for three (3) additional 12-month periods.



2. Double Churches Park Parking Addition ? RFB No. 18-0027

Scope of Bid

Provide construction services to expand parking at Double Churches Park. The

services include, but are not limited to, grading complete (including

demolition), asphalt paving, concrete sidewalk, drivable grass parking area,

parking lot striping, landscaping and storm drains.

1. Professional Surveying Services for Finance Department/Revenue

Division & Engineering Department (Annual Contract) ? RFP No. 18-0016

Scope of RFP

Provide the Finance Department Revenue Division professional surveying

services in conjunction with the application process for an Alcoholic Beverage

License, and provide surveying services for the Engineering Department in

conjunction with various projects, on an ?as needed? basis. The contract term

will be for two (2) years, with the option to renew for three additional

12-month periods.
