I. CLERK OF SUPERIOR COURT PERSONNEL:



Approval is requested to hire two Assistant Chief Deputy Clerks above Step C.

The stated positions are vacant and will be at a salary of $52,413.40, Pay

Grade 18-J. The Clerk of Superior Court has funding in its budget to support

this request and it will be effective on January 10, 2017.





II. ADDENDUM TO LEASE AGREEMENT - CCG EMPLOYEE HEALTH/WELLNESS FACILITY:



Approval is requested to execute an Addendum to the Medical Office Lease

Agreement for the CCG Employee Health/Wellness Facility with Physicians Realty

Trust that will allow extra space and necessary repairs in order to run the

facility efficiently.





III. COLUMBUS FIRE AND EMS - INOPERABLE SCHOOL BUSES MEMORANDUM OF

UNDERSTANDING:



Approval is requested to enter into a Memorandum of Understanding with the

Muscogee County School Board to accept transfer ownership of two inoperable

school buses. Columbus Department of Fire and EMS will use the buses in future

school bus extrication training. There is a $500.00 towing fee to have the

buses moved and funds are currently available in the Fire and EMS Medical

Services Budget.

1. Melrose Drive, Boxwood Boulevard & Decatur Street Bridge Replacements and

Waterford Road Bridge Improvements ? RFB No. 17-0028

Scope of Bid

Scope of the project includes removal and replacement of 3 bridges over Lindsey

Creek in Columbus, GA. The bridges are located on Melrose Drive, Boxwood

Boulevard, and Decatur Street. The bridges will have to be constructed

consecutively as detours for each bridge will have to use the others.



Work includes demolition of existing structure, complete installation of new

bridge, utility work as shown on the Contract Documents, and landscaping as

shown on the Contract Documents. Also included in this Bid is the replacement

of the bridge deck on Waterford Road as shown in the Contract Documents.



2. Landscape Improvements ? 2nd Avenue & 18th Street ? RFB No. 17-0029

Scope of Bid

The scope of work includes clearing & grubbing; erosion and sediment control;

planting of Emerald Colonnade Hollies, Muscogee Crape Myrtles, Shoal Creek

Vitexes, Carissa Hollies, Dwarf Buford Hollies, Pink Muhlies, Pizazz

Loropetalums, Coral Drift Roses, Dwarf Yaupons and Meyer Zoysia Sod; install

landscape mulch; and install an irrigation system.



3. Welding/Metal Fabrication Services (Re-Bid) ? RFB No. 17-0030

Scope of Bid

Provide welding and metal fabrication services for various facilities and

equipment owned by the City. The Term of the contract shall be for two (2)

years, with the option to renew for three (3) additional twelve-month periods.



January 18, 2017

1. Removal, Recycling, Reuse or Disposal of Mattresses & Box Springs from Pine

Grove Municipal Solid Waste Landfill ? RFB No. 17-0031

Scope of Bid

Provide services on an ?as needed? basis for removal, recycling, reuse or

disposal of mattresses and box springs for the Columbus Consolidated Government

Public Works Department. The term of the contract shall be for two (2) years,

with the option to renew for three (3) additional twelve-month periods.



2. Multi-Use Paper, Carbonless Paper and Envelopes for Print Shop ? RFB No.

17-0032

Scope of Bid

Provide white and color copy paper, carbonless paper, and envelopes, of various

weights and sizes, for use by the Columbus Consolidated Government Print Shop.

The items will be procured on an ?as needed? basis. The term of the contract

shall be for two (2) years, with the option to renew for three (3) additional

twelve-month periods.



3. Dispensers and Liquid Laundry Detergents (Re-Bid) (Annual Contract) ? RFB

No. 17-0025

Scope of Bid

Provide dispensers and liquid detergents for the laundry operations at the

Muscogee County Prison and Muscogee County Jail. Purchases of liquid

detergents will be purchased on an as needed basis by the Prison and the Jail.

The contract term will be for two (2) years, with the option to renew for three

(3) additional twelve-month periods.

1. Radar Certification for Public Safety Departments (Annual Contract) ? RFB

No. 17-0033

Scope of Bid

Provide radar recertification services for the 451 radars and 5 speed lasers

currently installed in the Columbus Consolidated Government Public Safety

Department Fleet. The contract period will be for one (2) years with the

option to renew for three (3) additional twelve-month periods.

Scope of RFP
