I. DONATION OF EXERCISE EQUIPMENT TO GALLOPS SENIOR CENTER:
Authorization is requested to accept one Reebok Treadmill V8.90 and one
Gazelle Freestyle Crosstrainer Glider from a senior who is a member of Gallops
Senior Center. An agenda report and resolution are attached.
II. FY16 FAMILY CONNECTION GRANT:
Authorization is requested to submit an application and, if approved,
accept a grant for $46,000, or as otherwise awarded, from the Georgia
Department of Human Services with no local match required and to amend the
Multi-Governmental Fund by the amount of the award. An agenda report and
resolution are attached.

C. Tasers for the Police Department
D. 3D Laser Scanner System for Crime Scenes

IV. FY 2016 ANNUAL ACTION PLAN (ADD-ON):
Authorization is requested for the filing of the FY 2016 Annual Action
Plan and applications for Entitlement funding for the Community Development
Block Grant (CDBG) and the HOME Investment Partnership Program (HOME). An
agenda report, resolution and annual action plan are attached.

1. Treatment Providers for Mental Health Court and Veterans Treatment Court
(Annual Contract) - RFP No. 15-0032
Scope of RFP
The Columbus Consolidated Government, Mental Health Court and Veterans
Treatment Court, is seeking proposals from qualified firms to provide treatment
to participants in the following areas: PTSD Counseling and Psychological
Assessment. The term of the contract will be for two (2) years with the option
to renew for three (3) additional twelve-month periods.
2. Automated Vehicle Location System (AVL) (Annual Contract) - RFP No. 15-0030
Scope of RFP
Provide a turnkey installation for a new AVL system for Department of
Transportation/METRA that matches and is functionally compatible with the
existing Motorola CAD/AVL system that is currently in use by the City?s E-911
Dispatch Center. The term of the contract will be for two (2) years with the
option to renew for three (3) additional twelve-month periods.

Scope of RFB
Construct the Torch Hill Road trail-head building and the Cusseta Road
trail-head building for the South Lumpkin Multi-use Trail.
