I. HISTORIC WESTVILLE INCORPORATION - LIVING HISTORY MUSEUM RELOCATION:



Authorization is requested to enter into a Memorandum of Understanding

(MOU) with Historic Westville, Inc., to relocate the Living History Museum to

the Oxbow Meadows site and authorizing conveyance of approximately 35 acres of

property, formerly known as the Orchard Grove Mobile Home Park, to Westville to

locate the Living History Museum and lease two tracts of land for economic

development and recreational purposes. An agenda report, agreement, map,

II. T-MOBILE ROOFTOP LEASE AGREEMENT:



Authorization is requested to execute a new Lease Agreement for

antennae and building space on the Columbus Consolidated Government Center

building, 100 10th Street, with T-Mobile South LLC., to operate and maintain a

telecommunications facility with annual rent of $15,208.75. The agreement will

be for 5 years with options to renew for four additional terms with a fixed 15%

III. FY2014 GEORGIA DEPARTMENT OF TRANSPORTATION - TRANSPORTATION PLANNING

CONTRACT:



Authorization is requested to execute a contract with the Georgia Department of

Transportation for Financial Assistance of $251,141.93 with a 20% local match

of $62,785.48 for a total of $313,927.41 to carry out the FY 2014

Transportation Planning Work Activities of the Columbus-Phenix City

Transportation Study (C-PCTS). An agenda report and resolution are attached.

IV. FISCAL YEAR 2012 HOMELAND SECURITY GRANT - MUSCOGEE COUNTY SHERIFF'S OFFICE:



Authorization is requested to submit an application, and if approved,

accept a grant in the amount of $3,500 with no local match, or that approved

amount, from the Georgia Emergency Management Agency for the Department of

Homeland Security Fiscal Year 2012 Homeland Security Grant Program/State

Homeland Security Program to provide supplies and equipment for the Explosive

V. 2013 HOMELAND SECURITY GRANT PROGRAM - COLUMBUS FIRE/EMS DEPARTMENT:



Authorization is requested to submit an application and ,if approved,

accept a grant in the amount of $26,000, or as otherwise awarded, with no local

match from the 2012 Homeland Security Grant Program for maintenance of

equipment received as part of a 2008 Hazardous Grant. An agenda report and

Emergency Purchase of Construction for Pipe Replacement Services at North

Highland:



On August 8, 2013, the Engineering Department requested emergency construction

services for pipe replacement at North Highland. During the investigation of

flooding at the North Highland Access Point construction, the Engineering

Department discovered a 36" high density polyethylene pipe that had partially

collapsed and other sections had holes. This is the pavilion area that is

currently under construction by Georgia Power. The pavilion was removed during

the construction of the weir structure required as part of the Whitewater

project. Uptown was also nearing completion of the cul-de-sac for the northern

Whitewater drop off. A sinkhole had previously been reported, but due to the

construction, was inaccessible for videoing. Since there are multiple

contractors in the area, and the construction zone is very small, Scott Bridge,

who is under contract with Uptown, was requested to provide a quote for the

pipe replacement.



The emergency construction services are needed to prevent further delays in the

cited projects, as well as, avoid additional conflicts by adding another

contractor to the already congested area. The City Manager approved the

emergency purchase on August 8, 2013. The estimated cost for the pipe

replacement is $43,560.00, any additional drainage material needed to backfill,

under concrete, will be an additional $32.00 per ton; other additional costs

may be incurred for various contingencies.

1. South Lumpkin Road Roundabout ? RFB No. 14-0001

Scope of Bid

Construct a roundabout at the intersection of South Lumpkin Road and Columbus

Water Works Road; provide approximately 3,500 linear feet of road pavement

improvements, including all drainage structures; provide pavement markings,

traffic signs, lighting, and landscaping; and provide traffic control and

warning devices.



August 21, 2013

1. Moon Road Widening ? Wilbur Drive to Whittlesey Boulevard ? RFB No. 14-0005

Scope of Bid

The work consists of widening Moon Road from Whittlesey Boulevard to Wilbur

Drive. The section of Moon Road is a two lane road and the project will widen

the roadway to three lanes, with turning lanes at street intersections. The

construction improvements also include all drainage structures, curb and

gutters, traffic signals, pavement markings, and other appurtenances.



2. Swim Gear (Annual Contract) ? RFB No. 14-0002

Scope of Bid

Provide various types of swimming gear on an ?as needed? basis to the Columbus

Parks and Recreation Division. The contract term shall be for two (2) years,

with the option to renew for three (3) additional twelve-month periods.

1. A Mandatory Site Visit is scheduled at 9:00 a.m. on Friday, August 23, 2013.

Attendees shall convene at the entrance of the Claflin School building, which

is located at 1532 Fifth Avenue, Columbus GA.



August 28, 2013

1. Forrest Road Bridges ? Cooper Creek and Bull Creek ? RFB No. 14-0007

Scope of Bid

The work consists of replacing bridges at Cooper Creek and Bull Creek located

on Forrest Road. In addition, the work includes some roadway improvements to

include all drainage structures, concrete curb and gutters, and asphalt

paving. The work also includes pavement markings; traffic signs and

signalization; and sidewalks.



August 30, 2013

1. Redevelopment Plan for Claflin School ? RFP No. 14-0002

Scope of RFP

The City of Columbus is requesting proposals from experienced, creative

architects, building and real estate development firms OR a philanthropic

individual or nonprofit group to present a redevelopment plan for adaptive

reuse of the Claflin School building that will be consistent with applicable

deed restrictions and preserve the historic site.



September 4, 2013

1. Weracoba Creek Retaining Wall Replacement ? RFB No. 14-0009

Scope of Bid

The work consists of removing and replacing a section of an existing retaining

wall located within Weracoba Creek and adjacent to Cherokee Avenue. The work

also includes pavement markings, installation of guard rails, and concrete

hardscapes work.

1. Schatulga Road / Corporate Ridge Parkway Roundabout ? RFB No. 14-0010

Scope of Bid

Construct a roundabout at the intersection of Schatulga Road and

Corporate Ridge Parkway. The work includes all drainage structures; concrete

curb and gutters; asphaltic concrete paving; and splitter islands. The work

also includes pavement markings, traffic signs, and landscaping.
