I. RIVER ROAD TOWER LEASE AGREEMENT:



Authorization is requested to enter into a 3rd Term Option Renewal -

River Road Lease Agreement with Verizon, Inc., and authorize the installation

of three additional Cellular Array antennas at location, 8889 River Road. An

II. MEMORANDUM OF AGREEMENT - COLUMBUS WATER WORKS:



Authorization is requested to enter into a Memorandum of Agreement with

the Columbus Water Works to perform variety of minor tasks and services that

are mutually beneficial. An agenda report, agreement and resolution are

III. EXECUTING AND FUNDING - PRE-CONSTRUCTION DRAINAGE IMPROVEMENTS:



Authorization is requested to proceed with executing and funding

pre-construction requirements for Drainage Improvements Projects funded through

the Local Option Sales Tax (LOST), Sewer Fund or Special Purpose Local Option

Sales Tax (SPLOST) to include execution of contract proposal from previously

approved on-call engineering/architectural/surveying firms, permanent and

temporary easements,utility relocation, environmental mitigation and other

outside agency agreements such as but not limited to Georgia Department of

Transportation, Railroads, Utilities, and other agreements to prepare projects

IV. STREET ACCEPTANCE - COPPICE DRIVE, PRAIRIE VALLEY DRIVE, PRAIRIE VALLEY

COURT AND EXTENSION OF GARRETT LAKE DRIVE LOCATED IN LAKES PHASE TWELVE,

GARRETT CREEK:



Authorization is requested for the acceptance of a deed to Coppice Drive,

Prairie Valley Drive, Prairie Valley Court and Extension of Garrett Lake Drive

located in Lakes Phase Twelve, Garrett Creek. The Department of Engineering

has inspected these streets and recommends the acceptance. An agenda report,

V. STREET ACCEPTANCE - EXTENSION OF SORREL DRIVE, EXTENSION OF SAN VISTA DRIVE

AND EXTENSION OF SORREL COURT LOCATED IN SECTION EIGHT, SONOMA POINTE:



Authorization is requested for the acceptance of a deed to the Extension of

Sorrel Drive, Extension of San Vista Drive and Extension of Sorrel Court

located in Section Eight, Sonoma Pointe. The Department of Engineering has

inspected these streets and recommends the acceptance. An agenda report, map

VII. FINANCE MONTHLY REPORT:

Emergency Purchases ? Information Only





Emergency Repair of HVAC at Government Center Annex:



On March 9, 2012, the Facilities Maintenance Manager requested the approval of

emergency repairs for the HVAC system at the Government Center Annex.



The repair was needed at the Annex because one system was down with a bad

compressor. This system controls the second floor of the Annex on the side

housing the Deputy City Manager?s office. Another system had a bad control

board and a leak in the piping, which required repair as well. With this

particular system, Facilities Maintenance must use R 410 refrigerant, which is

a blend refrigerant. Therefore when there is a leak, you must repair the leak,

evacuate the entire system and recharge. Facilities Maintenance used Holbrook

Service Company, because the vendor installed the system at the Annex. The

system at the annex is unique, and the only one of its kind in the area. The

cost for the repair is $6,440.00



The City Manager approved the emergency repairs on March 9, 2012.

1. Audit Services of Contract for Hospital Care to Indigent Persons ?

RFP No. 12-0014

Scope of Proposal

Columbus Consolidated Government is soliciting proposals for the professional

services of a Certified Public Accountant to perform a compliance audit of the

hospital care to indigent persons contract between the Columbus Consolidated

Government and the Medical Center Hospital Authority.



2. Course Instructors for the Department of Human Resources, Learning Center

(Annual) ? RFP No. 12-0022

Scope of Proposal

Provide Human Resources, Learning Center with instructors in various training

modules to employees interested in growing and developing their skills within

the Columbus Consolidated Government. The initial term of the contract shall

be on a per-course basis, up to one year, with the option to renew for one

additional year. Contract renewal will be contingent upon the mutual agreement

of the City and the Contractor.



3. Lead-Based Paint/Asbestos & Clearance Testing Services (Annual Contract) ?

RFP No. 12-0023

Scope of Proposal

Provide services for lead-based paint/asbestos and clearance testing services

for the housing rehabilitation, rental assistance program, and supportive

services/operations programs. The initial term of the contract shall be for

two years, with the option to renew for three additional twelve-month periods.

Contract renewal will be contingent upon the mutual agreement of the City and

the Contractor.

Scope of Bid

Provide Columbus Consolidated Government with various fastener supplies and

restocking services on an ?as needed basis?. Contract term shall be for two
