I. WESTERN REGIONAL CRIME LAB:



Authorization is requested to enter into an agreement with the Georgia Bureau

of Investigation to extend up to $66,525.00 for the specific purpose of

keeping the Western Regional Crime Lab open for the remainder of Fiscal Year

II. 10-YEAR HOMELESS PLAN - AMENDMENT TO LETTER OF AGREEMENT WITH THE

CUNNINGHAM CENTER FOR LEADERSHIP DEVELOPMENT:



Authorization is requested to amend the Letter of Agreement with the

Cunningham Center for Leadership Development (CLLD) to develop a 10-Year Plan

for the eradication of homelessness. An agenda report, letter of agreement,

timeline and resolution are attached.

III. STREET ACCEPTANCE - WOODDALE COURT, PORTION OF WOODVILLE COURT - THE

SECOND ADDITION TO WHITESVILLE WALK:



Authorization is requested to accept a deed to Wooddale Court and that

portion of Woodville Court located in the Second Addition to Whitesville Walk.

The Department of Engineering has inspected these streets and recommends

acceptance. An agenda report, map and resolution are attached. click link to

IV. ACCEPTANCE OF DETENTION BASIN - FIRST ADDITION TO WHITESVILLE WALK:



Authorization is requested for the acceptance of a deed to a 0.856 acres

Detention Basin located in the First Addition to Whitesville Walk. The

Department of Engineering has inspected this detention basin and recommends the

acceptance. An agenda report, resolution and map are attached. Click link to

V. STREET ACCEPTANCE - PORTION OF SPLENDOR WAY AND PORTION FOXHALL COURT -

SECTION THREE, BRIDGEMILL:



Authorization is requested for the acceptance of a deed to that portion of

Splendor Way and Foxhall Court located in Section Three, BridgeMill. The

Department of Engineering has inspected these streets and recommends the

VI. WOOLDRIDGE HEIGHTS SUBDIVISION - DESIGN VARIANCES:



Authorization is requested for denial of a request for design variances

for Wooldridge Heights Subdivision as described in the attached resolution

requested by Wooldridge Heights Development, Inc. An agenda report, map and

Scope of Bid

Provide various types of oil & lubricants on an ?as needed? basis to

the Columbus Consolidated Government (the City) to be utilized by METRA Transit

Department. The initial term of this contract shall be for three years, with

the option to renew for two additional twelve-month periods.



2. Paper & Plastic Products (Annual Contract) ? RFB No. 10-0050

Scope of Bid

Provide paper and plastic products, of various types and quantities, to

the Public Services department on an ?as needed? basis. The contract period

will be for two (2) years with the option to renew for three (3) additional

twelve-month periods.



March 25, 2010

1. Roof Replacement at Columbus Stockade ? RFB No. 10-0048

A Mandatory Pre-Bid Conference/Site Visit is scheduled at 11:00 AM (Eastern) on

Thursday, March 25, 2010 in the Conference Room of Muscogee County Jail, which

is located at 700 10th Street, Columbus, GA. Attendees should go to the main

entrance (front of the Jail) and ask to be directed to Administration. Vendors

will be allowed a 10-minute grace period. Any vendor who is not present in the

conference room within 10 minutes after the time slated for the beginning of

the mandatory pre-bid conference/site visit shall not be allowed to participate

any further in the bid process. Written requests for approval of any materials

submitted on this project as "Equals or Substitutes" for the specified material

must be submitted prior to or at the mandatory pre-bid conference/site visit.



March 26, 2010

1. Emergency Clean-up and Debris Removal (Annual Contract) ? RFP No.

10-0018

Scope of RFP

Provide services for emergency clean-up and removal of debris that may result

from natural (storm, fallen trees, etc.) or man made disasters (terrorism,

remnants of destroyed buildings, etc.) on an ?as needed basis?. The contract

will be for two years, with the option to renew for three (3) additional

twelve-month periods. Contract renewal will be contingent upon the mutual

agreement of the city and the Contractor.





2. Update of Pre-Disaster Mitigation Plan ? RFP No. 10-0017

Scope of RFP

The Emergency Management Division of the Columbus Fire and Emergency Medical

Services, Columbus Consolidated Government (the City) desires to update the

existing Pre-Disaster Mitigation Plan according to the requirements of the

Disaster Mitigation Act of 2000. The scope of work was designed in conformance

with FEMA Plan Guidance requirements.

1. Building Improvements for the Historic Train Depot (Re-Bid) ? RFB No. 10-0046

Scope of Bid

The work includes, but is not limited to, the installation of HVAC drain pans,

electrical fixture replacement, interior and exterior painting, carpet

replacement, and interior and exterior carpentry work. The City will only

consider bids from contractors with experience in restoration work for historic

structures.



2. Commercial Zero-Turn-Radius Mowers ? RFB No. 10-0045

Scope of Bid

Provide five (5) Commercial Zero-Turn-Radius Mowers. The mowers will be used by

the Right-of-Way Maintenance Division of the Public Services Department to

handle the Riverwalk Maintenance, as well as replace older mowers in our fleet.



April 7, 2010

1. Roof Replacement at Columbus Stockade ? RFB No. 10-0048

Scope of Bid

Provide all labor, equipment and materials to re-roof Stockade Building

designated flat roofs. Tear off existing BUR and replace with high performance

modified bitumen built-up roof system with flood coat and new pea gravel, new

metal flashing, new gutters and downspouts.



Written requests for approval of any materials submitted on this project as

"Equals or Substitutes" for the specified material must be submitted prior to

or at the mandatory pre-bid conference/site visit.



2. Building Improvements at Britt David Park ? RFB No. 10-0049

Provide building improvements at Britt David Park, which is located at 2700

West Britt David Road, Columbus, GA. Vendors may bid on one or both options:



Option 1: Remove and dispose (19) active and (2) stationary windows, and

furnish and install new horizontal metal sliding metal window units. Install

six (6) hollow core luan door units.



Option 2: Remove and dispose wood stairs and fabricate and install steel

stairs.

1. Grinding & Disposal of Yard Waste Material (Annual Contract) ? RFP

No. 10-0019

Scope of RFP

Operate a program to Grind and Dispose of Yard Waste Material for the Columbus

Consolidated Government. Contract term shall be for two years with the option

to renew for three (3) additional twelve-month periods. Contract renewal will

be contingent upon the mutual agreement of the City and the Contractor.

2. Provision, Installation, Testing and Service of LPG Propane Systems

and Vehicle Refueling (Annual Contract) ? RFP No. 10-0020

Scope of RFP

Turnkey installation to provide, install, test and service certified, LPG

(Propane) motor fuel systems and Propane for refueling of vehicles. The term of

this contract shall be for three (3) years with the option to renew for two (2)

twelve-month periods. The renewal of the contract is contingent upon the

mutual agreement of the City and the Contractor.



April 14, 2010

1. Storm Drain Rehabilitation ? Stimulus Rehab Contract II ? RFB No. 10-0052

Scope of Bid

The project consists of the following elements: The project consists of

performing cured-in-place storm sewer repairs as follows: approximately 150

feet of 12 inch diameter RCP pipe, 1,595 feet of 15 inch diameter RCP pipe, 440

feet of 18 inch diameter RCP pipe and 105 feet of 36 inch diameter RCP pipe,

and miscellaneous point repairs, camera inspection, and manhole adjustments.

The City will only consider bids from Pre-Qualified Contractors.



2. Lakebottom Park Walking Trail Improvements ? RFB No. 10-0043

Scope of Bid

Provide installation of park benches, trash receptacles, and concrete handicap

ramps. Additional work includes dumping, spreading and compacting M-10

screening material and replacing treated lumber used for trail edging.



All work shall be performed in accordance with the State of Georgia, Department

of Transportation ? Standard Specifications, Construction of Transportation

Systems, 2001 Edition or as Amended and in accordance with the contract

documents.
