I. DEVELOPMENT AGREEMENT - MEMORANDUM OF UNDERSTANDING (MOU) - VICTORY DRIVE:



Authorization is requested to enter into a Memorandum of Understanding with

Bright Meyers 2001, LLC for public transportation improvements along Victory

Drive associated with retail development as outlined in the MOU/Development

Agreement in accordance with Georgia Department of Transportation standards and

II. HEALTH & WELLNESS CENTER CONTRACT - TRANSFORM HEALTHRx OF GEORGIA:



Authorization is requested for a one-year renewal of the Health and Wellness

Center contract with Transform HealthRx of Georgia beginning January 14, 2015

through January 13, 2016. Funds are budgeted each fiscal year in the Employee

Health Insurance Fund. An agenda report and resolution are attached. Click

III. COLUMBUS FALL LINE TRACE - BIKE SHOP EASEMENT:



Authorization is requested to grant The Bike Shop, 3554 Hilton Avenue, a 60 ft.

easement to Columbus Fall Line Trace from their property. The Bike Shop will

be responsible for installing and maintaining a landscaped area long the Trace

in accordance with the Unified Development Ordinance as shown in the exhibit.

IV. COLUMBUS PARKS AND RECREATION - EMPTY BOWL EVENT:



Authorization is requested for the Department of Parks/Recreation to accept

donations in the form of gift certificates, checks, and cash from sponsors and

individuals for purchasing supplies before and during the Empty Bowl event as

well as for tickets to the event, auction pieces, bowls and monetary

donations. All funds (100%) will go to Feeding the Valley for the Kid's Cafe'

V. SHERIFF'S OFFICE - FY15 PUBLIC SAFETY RESERVE REQUEST:



Authorization is requested of a resolution transferring $211,819 of the FY15

Public Safety Vacancy Reserve for the Sheriff to the Sheriff's Office General

Fund Budget to fill 6 of the 7 vacant positions. An agenda report and

1. Plumbing & Irrigation Supplies (Annual Contract) ? RFB No. 15-0017

Scope of RFB

Provide plumbing & irrigation supplies on an ?as needed basis? to be picked up

and delivered to Columbus Consolidated Government. The contract term will be

for two years with the option to renew for three additional periods.



2. Aerial & Ground Ladder Inspection & Testing Services (Annual

Contract) ? RFB No. 15-0016

Scope of RFB

Provide aerial and ground ladder inspection and testing services to the

Columbus Fire & EMS Department. The contract period will be for two (2) years

with the option to renew for three (3) additional twelve-month periods.



3. Pavement Marking Services (Annual Contract) ? RFB No. 15-0019

Scope of RFB

Provide pavement marking service on an ?as needed basis? to Columbus

Consolidated Government to be utilized by Traffic Engineering Division. The

contract period will be for two (2) years with the option to renew for three

(3) additional twelve-month periods.

1. 457 Deferred Compensation Plan (Annual Contract) ? RFP No. 15-0004

Scope of RFP

The City of Columbus, Georgia is accepting proposals from qualified firms to

provide sole-provider administration, recordkeeping, education and investment

management services for the City?s Section 457 Deferred Compensation Plan.



The term of this contract shall be for two (2) years with the option to renew

for three (3) twelve-month periods. The renewal of the contract is contingent

upon the mutual agreement of the City and the Contractor.
