Authorization is requested for the Adult Felony Drug Court to enter

into a $50,000 contract with New Horizon for treatment services for

II. DEPARTMENT OF HEALTH/HUMAN SERVICES SUBSTANCE ABUSE AND MENTAL HEALTH

SERVICES ADMINISTRATION - GRANT:



Authorization is requested to submit an application and if approved, accept up

to $325,000 in funds from the Department of Health and Human Services Substance

Abuse and Mental Health Services Administration for the operations of the

Juvenile Drug Court Program. An agenda report and resolution are attached.

Authorization is requested to reimburse $198,111.25 in expenses to G C

W Development for the installation and maintenance of erosion control Best

Management Practices and Stabilization of a 25 acre site on Courtland Avenue,

where soil material was excavated by the City and utilized for cover at the

1. Storm Drain Rehabilitation ? Stimulus Rehab ? RFB No. 10-0036

Scope of RFB

The project consists of the following elements: performing cured-in-place storm

sewer repairs as follows: approximately 175 feet of 12 inch diameter RCP pipe,

260 feet of 15 inch diameter RCP pipe, 250 feet of 24 inch diameter clay pipe,

435 feet of 36 inch diameter RCP pipe, 495 feet of 36 inch diameter clay pipe,

335 feet of 30 inch diameter RCP pipe, 45 feet of 32 inch diameter RCP pipe, 45

feet of 18 inch diameter RCP pipe, and 330 feet of 18 inch diameter metal pipe.

Miscellaneous point repairs, camera inspection, and manhole adjustments. The

City will only consider bids from pre-qualified contractors.



2. Public Safety Building Exterior Waterproofing & New Gutters ? RFB No. 10-0037

Scope of RFB

Provide all labor, equipment and materials to perform roof maintenance and

exterior waterproofing to masonry walls, windows and designated mezzanine

decks. Install approximately 640 linear feet of new gutters and downspouts to

existing metal roof fascia. Written requests for approval of any materials

submitted on this project as "Equals or Substitutes" for the specified material

must be submitted prior to or at the mandatory pre-bid conference/site visit.



3. HVAC Equipment Upgrade for 911 Center ? RFB No. 10-0038

Scope of RFB

Provide all labor, equipment and materials to install one (1) complete 5-ton

system, including condensing unit, air handler, all ductwork and controls.



4. Electrical Services for 911 Center ? RFB No. 10-0039

Scope of RFB

Provide all labor, equipment and materials to install a new 100AMP 480V 3PH

power line from the electrical room to the new UPS system in the basement.

Install a 175AMP 208/120V 3 PH 42 circuit panel and sub-feed the existing panel

from the new UPS. Additionally, install a 20AMP 480V 3PH line from the

electrical room to the HVAC unit in the basement.

1. Retail Space for Park and Ride at the Warm Springs Road/Manchester

Expressway Locations (Annual Contract) ? RFP No. 10-0011

Scope of RFP

The Columbus Consolidated Government seeks a retail/service business to occupy

space within the new (Rails-to-Trails) Park and Ride locations. The building,

which will have the same footprint and layout, are currently being constructed

at Manchester Expressway and Warm Springs Road (See attached site plan).

Successful respondents will provide a service or combination of services that

meet the needs of the City and surrounding market area. While the City would

prefer that the retail services be oriented around trail uses. Suggested

retail services could include but are not limited to; bike rental and retail,

convenience store (no gas sales), newsstand or other similar service.



March 3, 2010

1. Full Size 4x4 Pickup Trucks ? RFB No. 10-0042

Scope of RFB

Provide three (3) Full Size 4x4 Pickup Trucks to be used by the Columbus Office

of Homeland Security.



March 5, 2010

1. Employee Physicals (Annual Contract) ? RFP No. 10-0005

Scope of RFP

Provide a cost effective service plan for providing pre-employment physicals,

periodic employee physicals, drug/alcohol testing and related employee health

services on an ?as needed basis?. The contract term shall be for two years,

with the option to renew for three additional twelve-month periods. Contract

renewal will be contingent upon the mutual agreement of the City and the

Contractor.



March 10, 2010

1. Warm Springs Bike Trail: Cooper Creek Bridge & Flat Rock Park Bridge

? RFB No. 10-0041

Scope of RFB

Provide construction of bridges at Cooper Creek and Flat Rock Park. The work

includes structural and site work in accordance with the designated plans and

specifications.



2. Lakebottom Park Track & Field Resurfacing and Trench Drain Installation ?

RFB No. 10-0040

Scope of RFB

The track and field resurfacing consists of providing a polyurethane running

track surface with structural spray finish, long jump/triple jump runway and

high jump area resurfacing and related work. The City will be responsible for

demolishing and removing the existing track.



The trench drain consists of installing a pre-cast, interlocking polyester

concrete trench drainage system around the entire track. The resurfacing

contractor shall be a member of the American Sports Builders Association.
