I. COLUMBUS GEORGIA 2025 ENDORSEMENT:



Approval is requested to endorse the Columbus Georgia 2025 Initiative.





II. PROPOSED PILOT PROJECT PARTNERSHIP BETWEEN CCG & RUBICON GLOBAL, LLC:



Approval is requested to accept a donation of consulting services and

technology from Rubicon Global, LLC, for a Municipal Waste Data Pilot Project

for services valued at $443,880.00 and authorize to enter into any appropriate

agreement(s) for participation in the waste data pilot project. The 12- month

partnership will allow the City to test a suite of technological resources

designed to improve residential waste and recycling systems.





III. PROJECT FRAMEWORK AGREEMENT FOR MOTT'S GREEN PLAZA (MONUMENT &

REHABILITATION):



Approval is requested to enter into a contractual agreement with the

Georgia Department of Transportation for the rehabilitation project for Mott's

Green Plaza in the amount of $1,000,000.00. Under this agreement, GDOT will

provide 80% of the Preliminary Engineering and Construction Cost ($800,000.00).

The local match of 20% ($200,000.00) is budgeted in the FY17 Capital

Improvement Program under the TSPLOST Discretionary funds.





IV. PROJECT FRAMEWORK AGREEMENT FOR INFANTRY ROAD & EXTENSION OF THE FOLLOW ME

TRAIL:



Approval is requested to enter into a contractual agreement with the

Georgia Department of Transportation for the new Infantry Road and the

extension of the Follow Me Trail project in the amount of $1,500,000.00. Under

this agreement, GDOT will provide 80% of the Preliminary Engineering and

Construction Cost ($1,200,000.00). The local match of 20% ($300,000.00) is

budgeted in Capital Improvement Program under the TSPLOST Discretionary funds.

V. STREET ACCEPTANCE - THAT PORTION OF FASHIONATA DRIVE LOCATED IN SECTION

THREE, OLD TOWN:



Approval is requested for the acceptant of a deed to that portion of

Fashionata Drive located in Section Three, Old Town. The Engineering

Department has inspected said street and recommends the acceptance. No city

funds are involved until maintenance is assumed after the two year warranty.





VI. 2016-1017 CRIME VICTIM ASSISTANCE GRANT:



Approval is requested to accept a Federal VOCA (Victims of Crimes Act)

Grant in the amount of $70,593 from the Georgia Criminal Justice Coordinating

Council and approved matching funding of $5,287 from the five percent crime

victim surcharge fund for the Solicitor General?s Victim Assistance Program and

amend the Multi Governmental Fund to reflect these revenues and expenses.

Scope of RFP

Columbus Consolidated Government (the City) is requesting proposals from

qualified firms to provide professional architectural, engineering and planning

services on an as-needed basis. The contract term shall be for two (2) years

with the option to renew for three (3) additional twelve-month periods.



2. Supplemental EMS Coverage (Annual Contract) ? RFP No. 17-0010

Scope of RFP

Columbus Consolidated Government (the City) is requesting proposals from state

of Georgia certified emergency medical services vendors to provide supplemental

advanced life support transport services, on an as-needed basis, in support of

Columbus Fire and Emergency Medical Services.

Scope of RFB

Provide trees to be planted on City right-of-ways for Columbus Consolidated

Government?s Department of Public Works. The trees will be purchased on

as-needed basis. The term of the contract shall be for two (2) years, with the

option to renew for three (3) additional twelve-month periods.



2. Tire Leasing Services for City Buses (Annual Contract) ? RFB No. 17-0027

Scope of RFB

Provide tire leasing services for City buses to the Columbus Consolidated

Government Department of Transportation/METRA Transit System, on an ?as-needed?

basis. The term of the contract shall be for two (2) years, with the option to

renew for three (3) additional twelve-month periods.
