I. FORREST ROAD IMPROVEMENT PROJECT - CONDEMNATION CORRECTION - 5768 FORREST

ROAD PARCEL 136 - AMENDMENT TO ADD DESCRIPTION:



Authorization is requested to amend Resolution #114-14 to include the

description of the Temporary Construction Easement for the acquisition of

Parcel 136 5768 Forrest Road (map attached) for the Forrest Road Improvement

Project in Muscogee County totaling 0.01 acres +/- of Right of Way, Temporary

Construction Easement totaling 0.01 acres+/-, plus two (2) Drive Way Easement

for County Road and/or Municipal Street purposes, and authorizing the filing of

a Declaration of Taking by the City Attorney or his representatives for the

acquisition of said land under the authority of the official code of Georgia

annotated, Section 32-3-1 et.seq for the referenced project. An agenda report,

II. FIRE DEPARTMENT - FY14 OTHER LOCAL OPTION SALES TAX CAPITAL REQUEST:



Authorization is requested to allocate $63,520 of the FY14 Budget-Other

Local Option Sales Tax Capital Reserve to purchase needed equipment (see

attached sheet) for paramedic training to remain in compliance with the Georgia

State Office of EMS and Accreditation and for 3 Low Light Cameras for Fire

III. 2014 GEORGIA COUNTY INTERNSHIP PROGRAM GRANT - ASSOCIATION OF CITY

COMMISSIONER OF GEORGIA - CIVIC AFFAIRS FOUNDATION:



Authorization is requested to submit an application and, if approved, accept a

grant for $2,000 from the Association of County Commissioners of Georgia Civic

Affairs Foundation for the Department of Human Resources to hire an Intern to

update forms used in the Records Management System for a ten week period,

IV. FISCAL YEAR 2013 HOMELAND SECURITY GRANT PROGRAM:



Authorization is requested to submit an application, and if approved, accept a

grant for $97,970, or the approved amount, from the Georgia Emergency

Management Agency for the Department of Homeland Security from Fiscal Year 2013

Homeland Security Grant Program/State Homeland Security Program, and amend the

Multi-Governmental Fund by the amount awarded. An agenda report and resolution

V. K9 HOMELAND SECURITY GRANT PROGRAM:



Authorization is requested to submit an application, and if approved, accept a

grant in the amount of $3,000, or the approved amount, from the Georgia

Emergency Management Agency for the Department of Homeland Security Fiscal Year

2013 Homeland Security Grant Program/State Homeland Security Program, and amend

the Multi-Governmental Fund by the amount awarded. Funds will be used to

purchase supplies and equipment for the Explosive Detection Canine Program. An

VI. 2013 HOMELAND SECURITY GRANT PROGRAM:



Authorization is requested to submit an application, and if approved, accept a

grant in the amount of $18,456, or the approved amount, from the 2013 Homeland

Security Grant Program to purchase equipment for the West Central Georgia

Search/Rescue (GSAR) Team. An agenda report and resolution are attached. Click

VII. GEORGIA RECREATION AND PARK ASSOCIATION INNOVATIVE GRANT:



Authorization is requested to submit an application, and if approved, accept a

grant in the amount of $1,000 or the approved amount, from Georgia Recreation

and Park Association (GRPA) for our "Hands in" Inclusive Therapeutic gardening

VIII. DRIVER POOL FOR INTEGRATED WASTE:



Authorization is requested of a resolution to establish a pool of temporary

drivers in the Integrated Waste Division through the Department of Human

Resources to be hired on an as needed basis as part-time employees only. An

1. Double Churches Tennis Court Lighting (Re-Bid) ? RFB No. 14-0037

A Mandatory Site Visit will be held on Tuesday, May 13, 2014 at 10:00 AM.

Vendors shall convene at the Double Churches Tennis Courts area, which is

located at 2300 Double Churches Road, Columbus, GA 31904. Vendors who attended

the previous site visit are not required to participate in the site visit on

May 13, 2014.



May 16, 2014

1. Wade Street Redevelopment ? RFP No. 14-0013

Scope of RFP

Columbus Consolidated Government is seeking proposals for the development of

1.35 (+/-) acres of vacant land located on Wade Street in South Columbus. The

property is located within the City of Columbus Enterprise Zone.



May 23, 2014

1. Traffic Signal Equipment (Annual Contract) - RFP No. 14-0018

Scope of RFP

Provide traffic signal equipment to Columbus Consolidated Government Traffic

Division on an ?as needed basis?. The contract term shall be for one year with

the option to renew for four additional twelve-month periods.

1. South Commons Park Memorial Wall ? RFB No. 14-0039

Scope of Bid

Work under this contract will include installation of three (3) free standing

brick walls to include precast concrete caps; installation of plastic script

lettering; and installation of four (4) aluminum plaques with bronze finish.



2. Double Churches Tennis Court Lighting (Re-Bid) ? RFB No. 14-0037

Scope of Bid

Provide sports lighting at the tennis courts and batting cage at Double

Churches Road Park.



June 4, 2014

1. St. Mary?s Road and Northstar Drive Roundabout ? RFB No. 14-0041

Scope of Bid

The work shall consist of constructing a roundabout at the intersection of St.

Mary?s Road and Northstar Drive, to include all drainage structures, concrete

curb and gutters, asphaltic concrete paving, and splitter islands. The work

shall also include pavement markings, traffic signs and landscaping. Special

attention should be given to the traffic control requirements.
