I. DIRECTOR OF INSPECTIONS AND CODES:



Approval is requested for the appointment of John Hudgison as Director of

Inspections and Codes. A resolution and resume are attached.





II. DIRECTOR OF FINANCE:



Approval is requested for the appointment of Angelica Alexander as Director of

Finance. A resolution and resume are attached.





III. TIA SUPPLEMENT AGREEMENT NO. 2 - US27/CUSTER ROAD INTERCHANGE

RECONSTRUCTION/MODIFICATION:



Approval is requested to enter into a supplemental agreement with the Georgia

Department of Transportation to adjust the budget for the Transportation

Improvement Act Project that will provide necessary funding for engineering

consulting services. There will not be any annual fiscal impact to the city

for this action and the total amount of $20 million in TIA funds for this

project has not changed.





IV. GEORGIA CHILD CARE AND PARENT SERVICES PROGRAM (GACAPS):



Approval is requested to sign a new Child Care Provider Agreement with the

Division of Family and Children Services that operates the Georgia Child Care

and Parent Services Program (GACAPS Program) which is designed to provide

eligible children with child care subsidies who attends camps and after school

programs at least once a week.





V. PRELIMINARY ENGINEERING CONTRACT - NORFOLK SOUTHERN:



Approval is requested to enter into an agreement with the Norfolk Southern

Railway Company to conduct preliminary engineering for the construction of a

overhead bridge at Buena Vista Road and Ace Way, at a cost not to exceed

$23,032.

VI. LEASE AGREEMENT - MARATHON PETROLEUM COMPANY:



Approval is requested to allow Marathon Petroleum Company to transfer all of

its rights, duties and obligations to MPLX Terminals LLC which are required in

the lease agreement with the City of Columbus. The City assumed this lease in

1996 as part of the Rails to Trails program. The City is not responsible for

any maintenance, expenses, or liabilities with this lease agreement and will

receive $15 per year for this right of way lease agreement. The gasoline

pipeline starts around 5030 Miller Road (Marathon Petroleum) and crosses the

Fall Line Trace at Miller Road.





VII. ROAD IMPROVEMENT, SAFETY AND ALTERNATIVE TRANSPORTATION PROJECTS:



Approval is requested to proceed with executing and funding the

pre-construction requirements for Road Improvement, Safety, and Alternative

Transportation Projects totaling $6,650,000 funded through the Other Local

Option Sales Tax (OLOST), Special Purpose Local Option Sales Tax (SPLOST),

Transportation Special Purpose Local Option Sales Tax (TSPLOST) Discretionary

Funds and State of Georgia, Department of Transportation Local Maintenance and

Improvement Grants (LMIG) to include right of way acquisition, permanent and

temporary construction easements, utility relocation, environmental mitigation

and other necessary agreements to prepare projects for construction and to

provide local match support to a grant submission led by The Community

Foundation of the Chattahoochee Valley, Inc. (CFCV) with already funded

projects in the amount of $3,000,000.

1. Construction Services for Bibb Mill Riverwalk ? RFP No. 16-0017

A Mandatory Pre-Proposal Conference/Site Visit will be held at 2:00 PM

(Eastern) on Tuesday, April 26, 2016. Vendors shall convene in the office of

the project architect, Hecht Burdeshaw Architects, Inc., which is located at 8

West 11th Street, Suite 300, Columbus, GA 31901. The Site Visit will commence

immediately after the Pre-Proposal Conference. Vendors will be allowed a

10-minute grace period for the Pre-Proposal Conference. Any vendor who is not

present in the aforementioned location, within ten (10) minutes after the time

slated for the beginning of the Mandatory Pre-Proposal Conference, shall not be

allowed to participate any further in the RFP process.

1. ?CM/GC at Risk? Services for City Mill Riverwalk ? RFP No. 16-0015

Scope of RFP

The Owner, by this Request for Proposals, desires to select a qualified

Contractor with the intent of both parties executing an Agreement for

Construction Services implementing the ?Construction Management/General

Contractor at Risk? project delivery method. The Owner has allocated a project

budget range of $1,100,000 to $1,500,000.



2. Advisory and Consulting Services for the Columbus Consolidated Government

457 Deferred Compensation Plan and Its Board (Annual Contract) ? RFP No. 16-0016

Scope of RFP

The Columbus Consolidated Government (CCG) seeks qualified firms to submit

proposals for providing professional advisory and investment consulting

services for the Columbus Consolidated Government?s 457 Deferred Compensation

Plan. The term of this contract will be for two years, with an option to renew

for three additional twelve-month periods.



3. Building Ethernet Wiring (Annual Contract) ? RFB No. 16-0031

Scope of Bid

Provide all tools and labor required to perform electrical services for

building Ethernet wiring at Fire Station #2 (1047 33rd Street), with potential

of many other locations in the next year. The contract period will be for two

(2) years with the option to renew for three (3) additional twelve-month

periods. Quotes will be obtained from the successful vendor, as needed, for

other locations.



May 3, 2016

1. Roof Replacement at Fire Station 4 ? RFB No. 16-0034

A Mandatory Pre-Bid Conference/Site Visit is scheduled at 11:00 AM (Eastern) on

Tuesday, May 3, 2016. Vendors shall convene at Fire Station No. 4, which is

located at 200 N. Oakley Drive, Columbus, GA 31906. Vendors will be allowed a

10-minute grace period. Any vendor who is not present at Fire Station No. 4

within 10 minutes after the time slated for the beginning of the mandatory

pre-bid conference/site visit shall not be allowed to participate any further

in the bid process.



Written requests for approval of any materials submitted on this project as

"Equals or Substitutes" for the specified material must be submitted prior to

or at the mandatory pre-bid conference/site visit.



May 4, 2016

1. Pool Chemicals (Rebid) (Annual Contract) ? RFB No. 16-0032

Scope of Bid

Provide water treatment chemicals to be used in the City?s swimming pools for

Park & Recreation on an ?as needed basis?. The contract period will be for two

years with the option to renew for three additional twelve-month periods.



May 11, 2016

1. Safety Equipment (Annual Contract) ? RFB No. 16-0033

Scope of Bid

Provide safety equipment to include: approximately 200 back support safety

belts, approximately 3,250 safety vests, approximately 5,000 safety t-shirts,

approximately 200 safety hats and approximately 3,000 safety caps to Columbus

Consolidated Government (the City) on an "as needed" basis. The contract term

will be for two years with the option to renew for three additional twelve

month periods.



May 13, 2016

1. Construction Services for Bibb Mill Riverwalk ? RFP No. 16-0017

Scope of RFP

The Owner, by this Request for Proposals, desires to select a qualified

Contractor with the intent of both parties executing an Agreement for

Construction Services for the proposed TIA project. The project will consist

of a new Riverwalk trail, approximately 2000 feet in length, connecting the

existing Riverwalk to the north and south.



2. Emergency Clean-up & Debris Removal (Annual Contract) ? RFP No. 16-0005

Scope of RFP

Columbus Consolidated Government is requesting qualified firms to submit

proposals to provide services for emergency clean-up and removal of debris that

may result from natural (storms, fallen trees, etc.) or man-made disasters

(terrorism, remnants of destroyed buildings, etc.) on an ?as needed? basis.



May 18, 2016

1. Roof Replacement at Fire Station 4 ? RFB No. 16-0034

Scope of Bid

Provide all labor, equipment and materials to install a new Thermoplastic

Polyolefin (TPO) roof system at Fire Station No. 4, which is located at 200 N.

Oakley Drive, Columbus, GA 31906. Written requests for approval of any

materials submitted on this project as "Equals or Substitutes" for the

specified material must be submitted prior to or at the mandatory pre-bid

conference/site visit.
