I. UNUSED SICK LEAVE:



Approval is requested to authorize a payment of $395,000 from the FY16

budget to employees for unused sick leave in accordance with 16B-15-6(2) of the

Columbus Code of Ordinances.





II. ROADSIDE ENHANCEMENT AND BEAUTIFICATION GRANT:



Approval is requested to apply for the Roadside Enhancement and

Beautification Grant in the amount of $32,656.50 for the purpose of landscaping

the northwest corner of 2nd Avenue and 18th Street and enter into a Mowing and

Maintenance Agreement between the City and Georgia Department of Transportation

with no local match required. The city would be responsible for the maintenance

of this site, which has been estimated at $8,420.

1. HVAC for Carver Park Recreation Center Gym ? RFB No. 16-0022

A Mandatory Pre-Bid Conference/Site Visit is scheduled for 10:00 AM (Eastern)

on Tuesday, November 17, 2015. Vendors shall convene in the gymnasium of the

Carver Park Recreation Center which is located at 6665 Hunter Road, Columbus,

GA.



November 18, 2015

1. Trailhead Buildings for South Lumpkin Multiuse Trail (Re-Bid)- RFB No.

16-0020

Scope of Bid

Construct the Torch Hill Road Trailhead Building and the Cusseta Road Trailhead

Building for the South Lumpkin Multiuse Trail.



2. Paper & Plastic Products (Annual Contract)- RFB No. 16-0019

Scope of Bid

Provide paper and plastic products, of various types and quantities, to the

Public Works department on an ?as needed? basis. The contract period will be

for two (2) years with the option to renew for three (3) additional

twelve-month periods.

1. Consulting and Insurance Broker Services for Property and Crime Insurance

(Annual Contract) ? RFP No. 16-0004 (RE-BID)

Scope of RFP

It is the intent of Columbus Consolidated Government (the City) to enter into

an annual contract with qualified offeror(s) to provide Consulting and

Insurance Broker Services (the Broker) to represent the City in various

insurance markets.



The initial term of the contract will be for one (1) year with the City?s

option to renew for four (4) additional twelve-month periods.



December 4, 2015

1. Waste Collection Services (Annual Contract) ? RFP No. 16-0007

Scope of RFP

Columbus Consolidated Government is seeking proposals from waste haulers to

provide waste collection services on an as-needed basis for Columbus, Muscogee

County, Georgia.



The initial term of the contract will be for five (5) years with the City?s

option to renew for five (5) additional twelve-month periods.

1. HVAC for Carver Park Recreation Center Gym ? RFB No. 16-0022

Scope of Bid

Columbus Consolidated Government is requesting bids to provide all labor,

equipment and materials to remove and dispose of two (2) existing gym package

units, and replace with two (2) gas package units at the Carver Park Recreation

Center Gym.
