Easement to BellSouth Telecommunications, Inc., d/b/a AT&T Georgia, south of

feet of 12-foot by 8-foot box culvert; 1,110 feet of twin 10-foot by 8-foot box

culvert; 820 feet of twin 12-foot by 8-foot box culvert; 1,280 feet of 14-foot

by 12-foot box culvert; 1,560 feet of 72-inch RCP gravity sewer; 1,110 feet of

60-inch RCP gravity sewer; 780 feet of 54-inch RCP gravity sewer; 630 feet of

Scope of Bid

Provide paper products and related supplies, which includes dispensers and

replacements on an ?as needed basis? for Columbus Civic Center. The contract

term shall be for two years with the option to renewal for three (3) additional

twelve-month periods. Contract renewal will be contingent upon the mutual

agreement of the City and the Contractor.

Scope of Bid

Provide one (1) 5,000 lbs. capacity forklift to be used by the Columbus Civic

Center at the new Ice Rink.



2. 2011 Ford Crown Victoria Pursuit Vehicles ? RFB No. 11-0038

Scope of Bid

This specification covers eleven (11) 2011 Ford Crown Victoria Pursuit Vehicles

to be used by the Sheriff?s Office. Ten (10) pursuit vehicles will be equipped

with police package and one (1) pursuit vehicle will be equipped with police

package (plain) with undercover light package. Do interior and storage space

needs, the public safety departments have deemed the Ford Crown Victoria the

only pursuit car that meets their needs in those areas.

3. Employee Service Award Pins (Annual Contract) ? RFB No. 11-0036

Scope of Bid

Provide employee service award pins to the Human Resources Department on an ?as

needed? basis. The contract period will be for two years with the option to

renew for three additional twelve-month periods.



4. Color Photocopiers (Annual Contract) ? RFB No. 11-0037

Scope of Bid

Provide, repair and maintain five (5) color photocopiers; one (1) for Planning

Department; two (2) for the Print Shop and two (2) for Geographic Information

System (GIS) located in the Government Center Annex. The contract period will

be for two years with the option to renew for three additional twelve-month

periods.

1. Moon Road from US 80 to US 27 Widening Project ? RFB No. 11-0034

Scope of Bid

The work shall consist of widening Moon Road from US 80 to US 27. The

contractor shall be required to maintain public access to all residences

located within the project area and maintain normal traffic.

1. In-Service Training Workshop Services for Community Schools Program

Scope of RFP

Provide Parks & Recreation various required in-service training for the

personnel in the Community Schools Program. The initial contract term shall be

for two years, with the option to renew for three additional twelve-month

periods.

1. Walking Trail Improvements at Lakebottom Park (Re-Bid) ? RFB No. 11-0039

Scope of Bid

Provide installation of park benches, trash receptacles, and handicap ramps.

Additional work includes dumping, spreading, and compacting M-10 rock screening

material and replacing treated lumber used for trail edging.
