CONSENT AGENDA
(1) 2ND READING:
Rezoning property located at 1111 Lockwood Avenue is proposed for rezoning from
RMF1 Residential Multi-Family 1) District to RO (Residential-Office) District.
(23-CA-05-Merritt) (Mayor Pro-Tem Rodgers) (The Planning Advisory Commission
and the Planning Division recommend conditional approval.)
(2) 2ND READING:
Rezoning property located on part of 7209 Williams Hill Road is proposed for
rezoning from RE1 (Residential Estate 1) District to SFR1 (Single Family
Residential 1) District. (22-A-05-Ford) (Mayor Pro-Tem Rodgers) (The Planning
Advisory Commission and the Planning Division recommend approval.)
(3) STREET ACCEPTANCE - PHEASANT LOOP CUL-DE-SAC LOCATED IN SECTION
THREE QUAIL CREEK:
Authorization is requested for the acceptance of a deed to Pheasant
Loop Cul-de-sac located in Section Three Quail Creek. The Department of
Engineering has inspected this street and recommends the acceptance. An agenda
report, map and resolution are attached.

(4) SURPLUS PROPERTY - BANKERS BOULEVARD:
Authorization is requested to declare the street segment and
right-of-way of Bankers Boulevard from its north right-of-way line south a
distance of 141 feet as surplus and transfer said property by Quit Claim to
TIC Credit Union in exchange for Parcel "A and "B" described by the attached
plat. An agenda report, map and resolution are attached.
(5) RESOLUTION:
Commending and congratulating the Brookstone School Lady Cougars tennis team
for winning their seventh Class A State Championship.
(6) RESOLUTION:
Commending and congratulating the Brookstone School Boys Cougars tennis team
for winning their first Class A State Championship since 1987.

Excusing Councilor R. Gary Allen from the May 24,2005 Council Meeting.

1. Credentialing & Security Access System ? RFP No. 05-0026
Scope of RFP
Provide a credentialing and security access system for the City, to be utilized
at the Government Center. Vendor will provide all equipment, software and
training for an employee identification credentialing and access system.
System must be capable of interfacing and communicating with the West Central
Health District and Columbus Fire Department current software and imaging
systems regarding data file transfers and communications with bar code scanners
to and from a central base database. The existing database for interface with
the Medical community is Card Control Systems, Inc., Tucker, Georgia.

1. Swipe Card Vending Machine? RFB No. 05-0073
Scope of RFB
Provide and install a swipe card vending machine to be used by the Metra
Transit System of the Columbus Consolidated Government. The swipe card vending
machine will accept fare pay and must have the ability to make change in both
bills and coins. It must also be compatible with the current GFI CentsABill
swipe cards and farebox system.
2. Weather Station? RFB No. 05-0079
Scope of RFB
Provide and install a weather station to be used by the Columbus
Fire/EMS Department.
3. Top Dresser (Re-Bid) ? RFB No. 05-0083
Scope of RFB
Provide one (1) turf top dresser to be used by the Park Services
Division.
4. Traffic Signal Contractor (Annual Contract) ? RFB No. 05-0080
A Mandatory Pre-Bid Conference will be held at 9:00 a.m. Wednesday, June 1,
2005 in the Government Center Annex Conference Room, 420 10th Street, Columbus,
GA. Vendors will be allowed a 10-minute grace period. Any vendor who is not
present in the Government Center Annex Conference Room within 10 minutes after
the time slated for the beginning of the Mandatory Pre-Bid Conference shall not
be allowed to participate any further in the bid process.
5. Breathing Apparatus (Annual Contract) - RFB No. 05-0084
Provide the Fire Department with self-contained breathing apparatus with
mask-mounted regulators, spare air cylinders, and repair parts. The contract
period will be for three years with the option to renew for two additional
twelve-month periods.

Scope of RFP
Contract with a qualified Traffic Signal Contractor who has the experience,
expertise and necessary equipment to perform traffic signal construction work
for the City on an annual ?as needed basis?. The contract term shall be for
two years, with the option to renew for three (3) additional twelve-month
periods, if agreeable to both parties.
