05/30/03 Auditor Howell Cone, conducted a site inspection to see if a

operating at location in question. The building is vacant

sign posted from Flournoy and Calhoun, Inc. Realtors.
Tax Supervisor, Yvonne Ivey contacted the realty office via
telephone and talked to
realtor Tom Calhoun who stated that the building had been
leased to Anthony
Hatchett for Puff?s Game Room. He stated that it was a 12
month lease and the
effective lease date is 06/01/03. Due to the repairs that
are needed on the building,
Mr. Calhoun stated that the rent was discounted.
06/02/03 Mr. Anthony Hatchett and Ms. Giowarnia Hatchett came to the
Occupation Tax
Section to apply for a business license. They were given a
Certificate of Occupancy
application and a Request/Renewal Form For A Business
License. Tax Supervisor
Yvonne Ivey explained to Mr. & Mrs. Hatchett that due to the
recent inquiries of
concerned citizens in the neighborhood of the proposed
location where they wish to
operate a game room, their applications will have to be
forwarded to other
departments within the Columbus Consolidated Government for
review before
processing a business license. Mr. & Mrs. Hatchett had their
lease agreement from the
realtors who listed the location in question as 58 31st
Street. Ms. Ivey called and
talked to Kevin Simms, a plans examiner in Inspection and
Codes who stated that the
correct address for the location in question was 58 31st
Avenue. In addition, auditor
Howell Cone confirmed the address to be 58 31st Avenue based
on the site inspection
that was conducted on May 30, 2003. Advised Mr. & Mrs.
Hatchett that they should
consult with the realtor to establish the correct address so
there would not be any
confusion relating to the location of the business
establishment. Mr. & Mrs. Hatchett
left to consult with realtor about correct address for
location and retained the
applications for the Certificate of Occupancy and
Request/Renewal Form For
Business License.
