I. DRAFT SHORT TERM WORK PROGRAM AND REVISED WASTE MANAGEMENT PLAN:



Authorization is requested to submit the Draft Short Term Work Program and

Revised Solid Waste Management Plan to the Lower Chattahoochee Regional

Development Center. An agenda report and resolution are attached.



II. 2003/2004 LOCAL ASSISTANCE ROAD PROGRAM

L.A.R.P. PROGRAM:



Authorization is requested to execute a Certificate of Ownership and Agreement

with the Georgia Department of Transportation for the 2003/2004 L.A.R.P.

Program for resurfacing 101 streets. An agenda report and resolution are

attached.



III. GEORGIA DEPARTMENT OF LABOR - HEALTH CARE RETRAINING PARTNERSHIP

INITIATIVE DEMONSTRATION GRANT:



Authorization is requested to accept responsibility as pass-through agency for

a Georgia Department of Labor - Health Care Retraining Partnership Initiative

Demonstration Grant that will be administered by the Lower Chattahoochee Valley

Workforce Investment Board. An agenda report and resolution are attached.



IV. COLUMBUS COTTONMOUTH'S LEASE AMENDMENT:



Authorization is requested of a resolution amending Section 8.1 of the

agreement with DVA Sports, L.L.C., a Georgia Limited Company d/b/a the Columbus

Cottonmouth's to increase concession rebate to the Cottonmouths from 25% to 50%

per event. An agenda report and resolution are attached.

V. MONTHLY 911 CHARGE:



Authorization is requested of a resolution reaffirming the necessity of the

$1.50 charge for 911 and the $1.00 charge for enhanced wireless 911 to continue

per billable telephone for another year. An agenda report and resolution are

attached.



VI. FY04 AMENDED HOLIDAY SCHEDULE:



Approval is requested of a resolution amending the FY04 Holiday Schedule to add

Veteran's Day as a holiday for the CCG. An agenda report, holiday schedule and

resolution are attached.

Scope of Bid

Provide uniform dress shoes & boots on an ?as needed basis? to various

departments of Columbus Consolidated Government (the City). To accommodate

walk-in service on some occasions, successful bidder shall have a business

located within a 25-mile radius of Muscogee County. The contract term will be

for one year, with the option to renew for two additional twelve-month periods.
