I. PUBLIC DEFENDER CONTRACT AMENDMENT:



Authorization is requested for an amendment to the Public Defender

contract that will allow travel reimbursement to be increased from $ 0.28/mile

to $0.485/mile as approved by the State of Georgia. An agenda report and

resolution are attached.





II. EASEMENT BRIGHTON PARK APARTMENTS:



Authorization is requested to execute a deed to Brighton Park

Apartments for a 20 ft. sanitary sewer easement through City property to

connect Brighton Park Apartments to the sanitary sewer line currently serving

Midland Chase Subdivision. An agenda report, maps and resolution are attached.

III. ENCROACHMENT ON RIGHT-OF-WAY 1043 3RD AVENUE



Authorization is requested to issue a permit to St. Luke Methodist

Church for an encroachment into the right-of-way for the installation of a new

building-facade along the west side of 3rd Avenue. An agenda report, maps and

resolution are attached.

A. Change Order No. 6 for Construction of Three Leisure Pools

B. Emergency Repair of Scales at Pine Grove Landfill

Provide stoneware clay to Columbus Consolidated Government, Parks &

2. Tri-Axle Low Boy Tractor ? RFB No. 06-0017

Scope of Bid

Provide one (1) tri-axle lowboy tractor to be used by the Street

Improvements Division of the Public Services Department.

1. Flood Abatement Project ? 12th Avenue System (Re-Bid) ? RFB No. 06-0018

Scope of Bid

The project consists of sewer and water system improvements including but not

limited to: approximately 2,200 feet of 48-inch reinforced concrete gravity

sewer, 1,200 feet of 42-inch reinforced concrete gravity sewer, 300 feet of

42-inch ductile iron pipe gravity sewer, 20 manholes, reconnection of sewer

service lateral and storm drain connections, approximately 3,100 feet of 6 to

12-inch ductile iron water main, valves, hydrants, service reconnections,

erosion and sedimentation control, and miscellaneous accessories.

1. Pest Control Services (Annual Contract) ? RFB No. 06-0021

Scope of Bid

Provide pest control services for eighty-six (86) City owned

buildings. Contract requirements include the control of pests (plus, mites,

ticks and spiders), rodents. The contract period will be for two (2) years

with the option to renew for three (3) additional twelve-month periods, if

agreeable to both parties.



October 26, 2005

1. Service Contract for Fire Extinguisher (Annual Contract) ? RFB No. 06-0023

Scope of Bid

Provide services for recharging, repairing and testing approximately

1,061 fire extinguishers at various locations and departments of the Columbus

Consolidated Government (the City). The contract period will be for two (2)

years with the option to renew for three (3) additional twelve-month periods,

if agreeable to both parties.

Scope of Bid

The work shall consist of the construction of Osceola Court in the Muscogee

Technology Park, in accordance with the designated plans and specifications.

The Contractor shall be required to maintain public access to all residences

and businesses located within the project area.
