(1) 2ND READING:



Regulating vehicular traffic in Columbus, Georgia by deleting from the

?Official Street Map, City of Columbus, Georgia? the street segment and

right-of-way of Bay Avenue from the north right-of-way line of Dillingham

Street north to south right-of-way line of Tenth Street; providing that all

existing utility easements of record remain in effect and providing appropriate

width as required by the applicable utility companies will be reserved for

unknown utilities; authorizing the disposal of said property in accordance with

the Columbus Charter; repealing any conflicting ordinances; and for other

purposes. (Councilor McDaniel)



(2) STREET ACCEPTANCE - KINGSBERRY STREET:



Authorization is requested to accept a deed to Kingsberry Street located in

Section Two, Moonlake Subdivision. The Department of Engineering has inspected

this street and recommends the acceptance. An agenda report, map and resolution

are attached.



(3) TEMPORARY STREET CLOSURE:



Application of Mr. Mike Snapp to temporarily close 3rd Avenue between

11th and 12th Street on Friday, April 2, 2004, from 7:00 a.m. to 2:00 p.m., in

connections with the "St. Luke School Field Day".

(a) Petition submitted by the Columbus Housing Initiative, Inc. to rezone

approximately 0.62 total acres of property located at 568, 602, 608 & 614

Chesterfield Avenue from a C-2 District to an R-3 District. (Recommended for

approval by both the Planning Advisory Commission and the Planning Division.)

Scope of Bid

Provide cleaning supplies to be utilized by various departments of the Columbus

Consolidated Government on an ?as needed basis?. The term of this contract

period shall be for one year, with the option to renew for two (2) additional

twelve-month periods. Contract renewal will be contingent upon the mutual

agreement of the City and the Contractor(s).

1. Forestry Management Plan ? RFP No. 04-0021

Scope of RFP

Columbus Consolidated Government (the city) wishes to obtain proposals to

establish forestry management plans for Carver Park (1825 Hunter Road), Cooper

Creek Park (4816 Milgen Road), and Flat Rock Park (6106 Warm Springs Road).

1. Medical Evaluation Services for Columbus Fire/EMS (Annual Contract ? RFP No.

04-0019

Scope of RFP

Provide medical evaluation services for the Columbus Consolidated Government,

Fire/EMS Department on an ?as needed basis?. The initial term of the contract

will be for two years with the option to renew for three additional

twelve-month periods. Contract renewal will be contingent upon the mutual

agreement of the City and the Contractor.

1. Ball Field Conditioner (Annual Contract) ? Bid No. 04-0068

Scope of Bid

Provide Columbus Consolidated Government, Parks and Recreation

Department with Turface MVP and Turface Quick Dry. The product is used to get

the ball field ready for play after it has rained. The term of contract shall

be for one year, with the option to renew for two (2) additional twelve-month

periods. Contract renewal will be contingent upon the mutual agreement of the

City and the Contractor.

2. Protective Fire Clothing (Annual Contract) ? Bid No. 04-0066

Scope of Bid

Provide firefighting with body protection against extreme temperature, sharp

objects, steam water penetration, hot particles, and other hazards encountered

during structural fire fighting. The contract term shall be for three years,

with the option to renew for two (2) additional twelve-month periods. Contract

will automatically be renewed on the anniversary date of the contract, unless

either party gives a 30-day written notice of cancellation.



3. Marquee Signs ? PQ No. 04-00012

Scope of Quote

Provide and install two (2) brick, LED marquee signs at Metra Transit

System. One marquee sign will be installed at the Main Office and one marquee

sign will be installed at the Transfer Center.



April 2, 2004

1. Stump Grinder? RFP No. 04-0020

Scope of Bid

The Columbus Consolidated Government (the city) desires to select a

qualified vendor to provide one (1) Heavy Duty Stump Grinder. The stump

grinder will be used by the Forestry Division of the Public Services Department

for grinding stumps from City rights-of-ways and other City properties.



April 7, 2004

1. Fastener Supplies & Restocking Services (Annual Contract) ? Bid No.

04-0058

Scope of Bid

Provide Columbus Consolidated Government with various fastener supplies

and restocking services on an ?as needed basis?. The quantities specified are

based on an estimated usage. The City may purchase some, all, more or none of

the item. The initial term of contract shall be for one year, with the option

to renew for two additional twelve-month periods. Contract will automatically

renew on the anniversary date of the contract, unless either party gives a

30-day written notice of cancellation.
