SPECIAL INSTRUCTIONS



This attached easement is granted pursuant to the following conditions:



1. The facility must be placed by bore method at 48? cover.

2. No clearing except that necessary for boring will be allowed on the property

without prior

approval from the City of Columbus.

3. The facility cannot interfere with the current use of the property.

4. Public Service Telephone will be responsible for replacement of any damaged

features

located on the property including, but not limited to, landscaping,

trees, fencing, drainage

facilities, etc.

5. The City will not be restricted on granting future easements.



Additional conditions applying to this project are as follows:



1. Proper site development permits are required.

2. There will be no activity under the drip line of the trees. This includes

storing of

materials, parking of equipment, digging entry holes for boring, etc.

3. Entry and exit holes for directional boring are to be as small as is

possible

for performing the necessary functions of the job.

4. All entry and exit holes for directional boring are to be filled in as soon

as possible.

5. No soil is to be put onto the roots under the drip line of the tree.
