(5) 2nd Reading ? Amending Ordinance No. 97-6, which rezoned properties bounded by JR Allen

Parkway, Whitesville Road and Bradley Park Drive, by amending Condition #2 to

allow for an additional, right-in/right-out only, curb cut.

(6) GEORGIA DEPARTMENT OF COMMUNITY AFFAIRS:

Authorization is requested to submit an application and if approved

accept a pass-through grant from the Georgia Department of Transportation,

Emergency Fund Grant for the Camp Robert Lewis in Fortson to assist with

construction of a bathhouse. An agenda report and resolution are included.



(7) FY2003 GRANT APPLICATION FOR STATEWIDE EARMARK CAPITAL ASSISTANCE:

Authorization is requested to apply and accept funds from the Federal

Transit Administration (FTA) and the Georgia Department of Transportation

(GDOT) for capital grants in the amount of $275,000 for the Department of

Transportation/METRA. An agenda report and resolution are included.

1. Chattsworth Road Improvements ? Bid No. 03-0009

Scope of Bid

Provide all labor, materials, tools, equipment and incidentals, and perform all

work required to widen Chattsworth Road as shown on the drawings, including

extension of storm drain pipes; and construct Litho-Krome Way (Road ?A?) as

shown on the drawings, including installation of storm drain and sanitary sewer.



2. Vans, Fifteen (15) Passenger ? Bid No. 03-0010

Scope of Bid

Provide three (3) new, current production model/year, fifteen (15) passenger

vans, with a minimum 8,500 GVWR, long wheelbase.



3. Midsize Extended Cab Pick-Up, Utility, 4 x 2 ? Bid No. 03-0007

Scope of Bid

Provide two (2) Mid-Size Extended Cab Pick-ups to be used by the Rainwater

Management Division and Waste Collection Division.



4. 7-Yard Dump Trucks with Cab and Chassis ? Bid No. 03-0011

Scope of Bid

Provide three (3); 7-Yard Dump Trucks With Cab and Chassis to be used by the

Sewer Maintenance Division of the Public Services Department.



5. Veterinary Services (Annual Contract) ? Bid No. 03-0013

Scope of Bid

Provide veterinarian services for the Public Services Department, Animal

Control Center. The contract term will be for three years with the option to

renew for two additional twelve-month periods.

6. Engineering Services Related to the Construction of Pine Grove MSW Landfill,

Phase 2, Cells 2A, 2B and 2C ? RFP No. 03-0007

Scope of RFP

Provide construction documents, construction administration and construction

quality assurance (CQA) services for the construction of Phase 2, Cells 2A, 2B

and 2C of Pine Grove Municipal Solid Waste Landfill. Services should include

relevant engineering and design, surveying, construction administration,

construction inspection, materials sampling and testing, preparation of

analytical reports, and all other reporting necessary to satisfy regulatory

provisions for CQA documentation.
