I. CHEROKEE AVENUE - 3-LANE ROADWAY:



Authorization is requested to convert Cherokee Avenue from the existing

4-lane roadway to a 3-lane roadway with a biking/pedestrian lane along the west

side between Garrard Street and Camille Drive, installation of a intersection

flasher at Cherokee Avenue and Slade Drive and realignment of Camille Drive

with Cherokee Avenue using stop sign control. An agenda report and resolution

are attached. (Delayed from the September 6, 2005, Council Meeting.)





II. FOUNDATION PROPERTIES, INC., GROUND LEASE AGREEMENT:



Authorization is requested to execute a Ground Lease Agreement with

Foundations Properties, Inc., at the annual rental of $120.00 per year for

thirty years and confirmation letter for Columbus Bank and Trust Company

regarding the ground lease. An agenda report and resolution are attached.





III. PUBLIC ACCESS DEFIBRILLATION (PAD) PROGRAM:



Authorization is requested to allow Columbus Heart Safe City, a

non-profit organization, to act as agent in a two-year agreement with

Medtronics Emergency Response System to initiate and implement a Public Access

Defibrillation (PAD) Program. An agenda report and resolution are attached.





IV. TRANSPORTATION ENHANCEMENT FUNDS:



Authorization is requested to submit an application and if approved

accept funds FY2006-2007 Transportation Enhancement fund from the Georgia

Department of Transportation for construction activities associated with the

Chattahoochee Riverwalk from the 13th Street Bridge to the 14th Street

Pedestrian Bridge. An agenda report and resolution are attached.



There will be a pre-council briefing Tuesday, September 20, 2005 at 8:30a.m. in

the Council Chamber Conference Room.

V. YOUTH AND THERAPEUTIC TENNIS PROGRAM GRANT:



Authorization is requested to submit an application and if approved

accept a grant of $3,000.00 from the United Stated Tennis Association (USTA)

foundation on conjunction with the NRPA step up to health Initiative with the

city. An agenda report and resolution are attached.





VI. STREET ACCEPTANCE- TO THAT PORTION OF BAYCREEK COURT AND MOYE PLANTATION:



Authorization is requested for acceptance of a portion of Baycreek

Court located in section four, Moye Plantation. The Department of Engineering

has inspected this street and recommends the acceptance. An agenda report, map

and resolution are attached.





VII. PURCHASES:



Approval is requested of items "A" and "B":

The project consists of sewer and water system improvements including but not

limited to: approximately 2,200 feet of 48-inch reinforced concrete gravity

sewer, 1,200 feet of 42-inch reinforced concrete gravity sewer, 300 feet of

42-inch ductile iron pipe gravity sewer, 20 manholes, reconnection of sewer

service lateral and storm drain connections, approximately 3,100 feet of 6 to

12-inch ductile iron water main, valves, hydrants, service reconnections,

erosion and sedimentation control, and miscellaneous accessories.
