I. STREET ACCEPTANCE - EXTENSION OF CREEKLAND DRIVE AND DETENTION POND:



Authorization is requested to accept a deed to the extension of Creekland Drive

and Detention Pond in Sections Four and Five, Big Creek Subdivision lying in

Land Lot 61, 8th District. The Department of Engineering has inspected these

streets and detention pond and recommends the acceptance. An agenda report, map

and resolution are attached.

1. Emergency Medical Replenishable Supplies (Annual Contract) -

Bid No. 03-0101

Scope of Bid

Provide various emergency medical supplies on an "as needed basis" to include:

Airway Supplies, Intravenous Supplies, Oxygen Delivery Supplies, Bandages,

Exposure Control Supplies, Exam Gloves and other Miscellaneous items. The term

of the contract shall be for one (1) year period, with the option to renew for

two (2) additional twelve-month periods, if agreeable to both parties.

1. Group Life, AD&D and Long Term Disability Insurance (Annual Contract) -

Bid No. 04-0002

Scope of Bid

Provide Columbus Consolidated Government employees with Group Life, AD&D and

Long-Term Disability Insurance through a fully funded program. The term of

contract shall be for two years, beginning September 1,2003, with the option to

renew for three (3) additional twelve-month periods. Contract renewal will be

contingent upon the mutual agreement of the City and the Contractor.
