year; specify New Horizons as the sole provider for mental health services;
grant authority to the Mayor to execute all documents and amend the
Multi-Governmental Grant
Fund.
Currently, we have a grant for Mental
Health Court through the Bureau of Justice Assistance which will expire in
September 2005. This new grant is an expansion of the first grant. The goals of
this grant are to: divert persons with mental illness and/or co-occurring
substance abuse disorders from jails to community based mental health; provide
directly, or indirectly, treatment services that are based on best known
practices; and promote development of a comprehensive service delivery system.
The current grant has assisted over 85 participants in State Court and Superior
Court. New Horizons staff members assess persons in the jail and make
recommendations regarding who can be diverted from additional jail time to
treatment services. Individuals admitted to Mental Health Court then receive
intensive case management services, and report back to the court regarding
their progress and compliance with treatment. This next phase will focus more
on planning, evaluation and developing more jail diversion activities. The Jail
Diversion Planning Committee will grow out of the current Mental Health Court
Steering Committee, but will be enlarged to include more stakeholders.
Following the planning period, the model(s) selected by the Planning Committee
will be implemented. The major goals of the grant would be to be able to
provide case management services to approximately twice the number of consumers
currently served through Mental Health Court (i.e., about 160 consumers at any
given time), to include a ?specialty? case manager that would focus on women?s
issues (which often include DFACS and other agencies) and to plan and implement
additional diversion activities. The scope of the additional diversion
programs would be determined by the Planning Committee. All programs will be
evaluated for effectiveness to ensure that they are working as planned, and to
evaluate the savings to the jail.

Analysis

This grant is scheduled to extend over a 3 year period on a reimbursable
basis. The primary provider of the service will be New Horizons. This will
require that they be the sole provider of the contractual services rendered if
the grant is approved. New Horizons may subcontract additional service
providers. Although this grant does not demand that the program be sustained at
the end of the grant period, a plan must be submitted from the grantee on
efforts of sustainability in year 1.

Financial Considerations

This grant will be a reimbursable grant which would begin October 2006 through
September 2009. The grant is a 75/25 proposition with the Federal share of
up to $400,000 per year. The local match of 25% ($100,000) will be
provided from in kind services from New Horizons. The City will be the fiscal
agent for the grant. A memorandum of understanding will be completed with the
agency on the responsibilities of each entity.
